So since I’ve been on estradiol for 22 months and my body produces very little testosterone, my value for T. Is around 100 without blockers, I have gone down to near zero and my body didn’t adjust well, so T is back up some and so with these levels should I consider myself as female for chemistry purposes with vitamins and supplements? Doses are different for male and female. Or should I still say I’m male and go by the male recommendations? Just looking for insite on how to consider myself. I do fully realize I am a trans woman and not a biological woman. Any advice would be greatly appreciated,
I generally aim for "cis female levels" of estradiol and testosterone for a trans female. In the lab I use, cis female testosterone runs to about 55. I find that when people have very low T levels, they may have lower libido and also lower energy in general - not necessarily a desired outcome. I'm not sure about the male and female doses for vitamins - so you might want to ask the pharmacist. Hope this helps. Linda

Can this work after so many years of minimal development? I have tuberous breasts. I recently undergone BA and im not sure if i want to keep them for years. I would love to have growth if i explant them
My best recommendation is to consult your surgeon - or another surgeon who is experienced in breast augmentation for transgender women. They will be more qualified to answer - and in your particular situation - than I could possibly be. Good luck and thank you for your question. Linda

I noticed something very interesting concerning HRT. I had been on Spironolactone and estrogen for less than six months. I was having hip pain which felt to me like my hip joints were trying to rotate. I was taken off the hormones not for that but because I was having water retention issues and another issue. After being off the estrogen for one month or so, not quite sure how long. The doctor agreed to let me use a very small amount of testosterone cream for a short period of time to help jump start my testicles and penis. What the interesting part was my nipples and penis were reacting to the testosterone as if it were estrogen. My nipples started to swell and my penis relaxed and retracted into my body like it was doing when I was on estrogen. I thought that maybe the parts of my body that had received the estrogen could no longer distinguish between estrogen or testosterone. I don't know if that is even physically possible. But I understand now it was likely just a decreased sensitivity to testosterone or a conversion into estrogen. I was quickly off the testosterone which did its job as my sexual function recovered much quicker than it would have otherwise. Thank you for the interesting videos.

@@lindagromkomd I did some more investigating. It can be about the supplemental testosterone being high. Because the higher it is more can be converted to estrogen. Some men do convert testosterone to estrogen at a higher rate than others. Also having more sensitivity to hormonal changes can cause the nipple area to react to it. And yes it seems that preexisting breast tissues can be more responsive to estrogen when it receives it such as in the case where the testosterone is converted to estrogen.
This was very interesting. It reminded me of the time in my twenties when I put my semen on a slide and looked at it under my microscope. There were so few visible sperm that it suggested to me that I was close to being infertile. The sperm could be clearly seen so it was not a focus issue or improper use of the microscope. In my mind I am transfeminine. I have ADHD and low oxytocin. I had premature birth, delayed puberty, underdeveloped male genitalia and hormonal resistant issues. I have blue eyes and a very rare blood type. My eye color helped me notice an interesting coincidence. The sample was small of course. But in watching videos of transgender women and men the great majority of them had blue or gray eye color rather than brown. So that did seem rather interesting. Just recently I started reading about different problems or medical issues that transgender people have and it was interesting to see that they can have many issues in common. Just this week I recalled the time when I was very young. About 3 and 1/2 years old. I was dressed in a little suit so I looked like my dad. At the time I remember being happy because I wanted to be like my father. But even at that young age I felt inauthentic dressing like him. I never felt that I was a girl back then. But I felt like I was partially molded clay that the artist started to mold but never finished. I guess a saving grace was that my body was slim hairless,. genitals small and often I was perceived as being feminine. That helped ward off general feeling of being dysphoric. My main dysphoria was a feeling that my female breasts should have developed and they never did. I do feel some minor level of stress all the time. But I perceive it as not dysphoria due to brain body mismatch. But I attribute it to the fact that I know I will never be 100% accepted for who I am. I can happily say that I have achieved a level of peace despite my condition and live a generally happy life.

@lindagromkomd In my haste of making my original comment I forgot to add that my body finally started producing body hair between the ages of 26 and 30, but there was no further maturation of the genitals. For that reason I suspect my chromosomes are likely normal. And all my issues are hormone related. The chromosomal testing is a good idea and I thank you for the idea. What is a strange concept to me though is how your body can be producing all the necessary hormones but be partially resistant to them which I'm sure is what is happening in my case. I did have a particular blood test a long time ago. I also have two lesbian cousins from my mother's side which suggests to me hormonal issues in the womb for them as well. Just recently I found the word gynosexual. I never heard of the word before. The reason I found it was I was looking up the reasons why seemingly straight males were attracted to me often in a sexual manner . This happened as early as in the 9th grade and continued on to my late twenties. Having males attracted to me was very confusing because I didn't identify as gay. But now I realize they were attracted to my femininity or what they perceived as feminine. A friend suggested blood work too. But to me I don't see much sense in it due to the lineage cessation that will happen with our family line. It's like we came to the country just in time to fight world War II. But now that we are not needed we'll leave as quickly as we came.
What's the safe dose to starting?
Dosing depends on several factors, including route of medication (patches, pills, injections). A good resource for this is transcare.ucsf.edu/guidelines/feminizing-hormone-therapy. But the best advice, of course, is to talk with your medical provider. Linda

Out of curiosity, AMAB, I had to have my gallbladder removed for obstructive stones a few years ago but have recently started estrogen for GD (worked amazing I might add). My question is; should I be concerned about having further complications related to the bile duct and estrogen use giving my past history with gallstones?
Hello. If you have had your gallbladder removed, you won't likely have to worry about stones. Very rarely, we see the development of a stone in the residual portion of the duct that is left after surgery - but it is highly unlikely. I'm glad you are liking the effects of estrogen. Linda

Thank you for your comment. Many AFAB individuals experience a great deal of "bottom growth" or clitoral enlargement with testosterone. Because the penis and clitoris are the same structures embryologically, the enlarged clitoris can look just like a small penis. The vaginal opening remains as is. I started Spiro and estrogen in 2018 along with methadone after 2 years iv been suffering from fluid retention iv been off HRT now 6 mths is there any i can do to start HRT again without it effecting my health im still getting fluid retention and on methadone i just want to be happy i was diagnosed with gender dysphoria since an early age around 5 or 7 years old
Thank you for your comments. You will want to talk with your doctor about methadone and fluid retention. It is possible that spironolactone - because it is a diurectic and makes you urinate more - may help with the fluid retention. Unfortunately, I cannot answer what is/is not safe for you personally. Greetings, the answer depends on a number of factors. If you stop hormones, you might notice certain changes: possibly hair growth, skin changes. None of your surgical changes would reverse, of course. I always recommend that people who are not on hormones get their hormone levels checked - and get a DEXA scan to see how your bone density is doing. Bone density is improved with estrogen or testosterone. And decreased bone density can lead to osteoporosis and fractures. But you would clearly want to discuss this with your healthcare provider. How small can the pe*is shrink? What's an extreme case? Also, what would be an extreme case of hip and bre*st enlargement? What would be considered the "outer limits" of these changes?
Hello. I think it would be fair to say that penile and prostate shrinkage that occurs on estradiol is subtle - not dramatic. But I think it's fair to warn patients about this. In my experience, hip changes are subtle also. Breast increase with estradiol varies considerably. I remember one woman who was developing so quickly she worried she'd be outed, fired, and lose her pension. (Everything worked out well!) Clearly, body shape changes are most dramatic with body contouring or augmentation surgery. Linda
A lot of the shrinkage isn't really shrinkage in a classic sense in regard to the penis. What often happens as I describe it is the penis goes into a very relaxed state and retracts into your body. So your flaccid penis can look like it lost an inch in length. The testicle shrinkage can vary really widely. For example some might have noticed their testicles only lost a quarter of their volume, while another person may have lost 1/2 of their volume.
Oral T can hurt your liver. My endocrinology doctor is a WPATH executive. He told me the oral is dangerous. Please tell people to read the side-effects. Oh, I'm a transman.

I'm 19 years old, i started my transition one month and 2 weeks ago. I started with cyproterone acetate for blocking testosterone (androcur) 12.5mg. But literally one week after starting taking the pills, i felt dizzy and my blood pressure got high without any reason.. my endocrinologist said that after taking all the cyproterone acetate pills i'm gonna start taking spironolactone pills (100mg). do you think spironolactone would help reducing the blood pressure in my case? I'm actually confused about what just happened 'cause i'm too young to start experiencing high blood pressure.. and it is so weird to be experiencing this things one week after starting taking testosterone blockers and i haven't started taking estradiol yet.
Greetings, I'm having difficulty thinking that I already responded to you. If I did not, I apologize. I'm not sure why your BP went up on cyproterone acetate. With spironolactone, I sometimes see BP going down - it is a blood pressure medication after all. I was born with a sexually development body often called intersex. My right testis failed to develop and did not drop and at ten years old surgery for a undescended testis was done. Apparently some ( 2-3) teratoma were found and removed during this surgery but were not reported to me at any time after the surgery. My parents claimed they did not know, I did not believe them when years later I found out that I had this condition. As an adult this same testis started to grow but it took me nine months of trying to get any doctor to do any basic testing and even when that was done it was done incorrectly. Finely I demanded this testis be removed and a local surgeon removed this testis though my lower abdomen while I was wide awake. When he cut me a 10cm malignant germ cell teratoma poked out of the wound. He kept injecting pain killers just ahead of where he was cutting to remove the mass from the abdomen and scrotum. He used surgical mesh and tape to put me back together but this all ripped out two days after surgery. A real mess. I needed seven months of chemo and additional surgery at the Mayo Clinic for one other 17.5 cm teratoma in my upper abdomen. My life was hell for years after, It was found that this particular testis was in fact a fetal gonad, teratoma and my cancer was related to ovarian cancer. My remaining testis Is normal but never matured to normal adult size. I did marry but clearly I did not sexually mature as most male but we managed to make our marriage work for us. I have been treated for gender dysphoria but wouldn't call myself transgender in the classic sense of the word. I have also given a great deal of thought of transitioning to female as I have never seen myself as purely male at any time in my life. Between surgery, cancer and chemo there isn't much male in me any more. The sad truth is my cancer took the form of ovarian cancer thus taking estrogen hormones could cause the cancer to return. Have you ever encountered someone with the situation Ive been in. ??

This is a great fear among trans people and everyone who cares about them. Our best assurance is to vote, of course. I am trying to encourage everyone to register now to be able to vote in the November election (www.vote.gov). If Trump were to be elected, I am guessing there would still be safe states for surgeries and hormone prescriptions. This is far from optimal, but it's something. I appreciate your fear. Linda

Next, my girl friend offered her Birth Control pills to me for a few months. The pills worked wonders. I finally started prescription Estrogen about September 1979. I began studying phyto-Estrogen in 1981. That also works for me. *
Thanks for your email. I always try to pass on the warning not to take birth control pills as your form of estrogen. The estrogen in birth control pills (ethinyl estradiol) has a higher blood clotting risk than estradiol we use for gender affirmation. I'm glad things are working well for you now. LG

Are you referring to a non-binary surgery that retains the penis and scrotum and creates a vagina? I haven't heard of that. Please tell me what you know. Linda
@@lindagromkomd Yes exactly that. on the "queerdoc" website and a few others they mention that the testicles AND scrotum may be kept, although this reduces the amount of tissue available for vulvoplasty, and the total amount of real estate available for the vaginal opening. Based on this, I am trying to understand where the vagina would go/how it would work. Apparently it is self-lubricating as well.
Interesting. I know that when more external tissue is needed, a skin graft can be taken from the waist area - bilaterally - so that scars blend in with the skin lines. As far as the internal tissue would go, I wonder if this would be a circumstance for peritoneal pull-through vaginoplasty; that would provide for lubrication. The right colon vaginoplasty also provides for self-lubrication. I see your point about limited real estate for the vaginal opening. Also, please remember, I'm not a surgeon! Linda

Do you know of any doctors that provide vestibuldectomy for provoked vulvar vestibulitis in combination with a vaginectomy for gender affirming surgery? Can I get both surgeries done at the same time by the same surgeon? I prefer a woman surgeon due to trauma. That's a combo surgery I hope to get for myself. I don't want a phallus. Just want to not be in pain and be comfortable in my non-binary body which I am not sexually active in and expect to never be active as I am an asexual human trafficking survivor who was born into slavery and trafficked for 35 years. I am safe now and 41 years old. I'm fine with getting a hysterectomy first, as I have a uterine septum and I had this really painful and active fibroid that might have gone away with my use of birth control pills to control the excessive bleeding. Anyway, I am asexual and I always have been and will be forever. I like it. I like my celibate life. So can you please refer me to a surgeon who can do both a vaginectomy and a vestibuldectomy on the same day for me, and sew up the hole so I just have skin there, without neuroproliferation created by trauma that hurts when provoked? I have goals to become a horseback rider and be able to put my pelvis into a rock climbing harness so I can enjoy mountain climbing, high ropes and zip lines, which I cannot currently do because of my extremely annoying vulvar vestibulitis. It's way too painful.

However, I would recommend looking for surgeons who do gender-affirming care with a track record of doing non-binary surgeries.
