Fasciculations (twitchings) of muscles of thighs and tongue seen in a patient with MND (motor neutron disease), or Amyotrophic lateral sclerosis (ALS) which a subtype of MND. It's a disease of anterior horn cells (motor neurone) in the spinal cord (LMN or lower motor neuron lesion).

25 yr old lady with severe hypocalcemia presented with tetany, Trousseau sign and Chvostek sign. Trousseau sign - carpopedal spasm elicited by inflating BP cuff to 20mm Hg above systolic BP for a few minutes.

Also, there may be fanning of the other toes. This suggests that there is been spread of the sensory input beyond the S1 myotome to L4 and L5.
