The ESC/EAS guidelines also mention, that coronary arterial calcification can be used to diagnose your risk (p 126-7). Having LDL of more than 200 mg/dl (5mM) since at least 20 years, my general practitioner suggested the initiation of a statin therapy. We agreed to make the decision dependent on the outcome of a heart CT. It turned out, that at the age of 67, my CAC score was well below the 50% percentile of my age group.I kind of expected this result, having a BMI of 27, daily exercise and no cardiovascular risk in the family. Ironically, my attempts to avoid any regular medication was flouted, when I had my first attack of gout 3 years later, despite (high-) normal uric acid levels.
Based on guidelines, an LDL >190 should be on cholesterol lowering therapy. Generally we don’t use CACS to risk stratify these individuals because they should be on lipid lowering therapy regardless of calcium score. As I said in this video, meds aren’t a cure but those with LDL that high are at a much higher long-term risk. Ultimately decision is between you and your doctor and being a low risk individual is certainly better than having multiple cardiac risk factors. But based on guidelines, LDL >190 in most patients should be treated
Thank you. Super helpful info. Oddly, my symptoms began with my COVID 60 days ago... and my Doc prescribed a low dose of prednisone. Oops. Which kept me up all night with heart palpitations. I think that's where it began. Doctors still really suck at diagnosing this condition. Been i7-8 level npain for months and all I needed was two Advil? You gotta be kidding me?
I think diet is indivisual specific ,effect of diet depends of how your body is constructed to react and consume and burn that food that you consume....
At 50, and refusing a statin prescription for ldl at 120,with 59 Hdl and 70 triglycerides had a CAC of 0, pcp finally relented and said just keep doing what you're doing.
@@brianf761 That’s great! CACS of zero is one of the every few times we can confidently say “you don’t need meds!” (Assuming LDL not >190 mg/dL)

@marcKatzmd Great Video. I have been hearing how current thinking indicates that Triglycerides are more important than LDL because Triglycerides become the much more dangerous VLDL. Would love to hear your thoughts on this.
I have a new video coming out soon on cholesterol and will answer this question in greater detail but generally LDL, ApoB, and LpA are more important than TG. Persostently elevated TG are not to be ignored so I certainly care about TG but LDL is still largely a greater focus

I hope you address these two issues (1) The Framingham heart study showed a 30% increase in CVD associated with high LDL but an 80% increase in CVD due to TG 2) LDL (as you stated) is correlated with increased CVD but there is no causation rationale. The folks who believe TG is the issue believe that inflammation caused by sugar, smoking, and alcohol attracts the LDL as the LDL is trying to heal the artery. Blaming the LDL for CVD is like blaming the scar for the knife wound.

Very confident doctor. I have a few questions: What if Crp reduces but Esr remain in 40s? Is a thin film of about 3mm of PE of any significance? Does the patient keep on repeating 2-D echo to see that quantity of fluid is not increasing? If so how often? Any other diagnostic tests? Lastly can the fluid go away in its own ?
While AI can help physicians with a variety of activities, it cannot take the place of the human touch and knowledge that are necessary to deliver high-quality medical care.

@@MarcKatzMD and that is where you have gone wrong! Demonizing saturated fat has been the biggest health blunder. Statins are one of the most widely prescribed drugs and heart disease is going up not down. LDL is NOT the cause. Insulin resistance and inflammation is.

Sir i am bit little confuse I've heard surgeons are physicians to so when a cardiac surgeon can do surgery and prescribe medicine than why are cardiologist made , like when a general surgeon can do surgery and gives medicine for general disease than why internal medicine doctors are made

what bothers me the most is the fear that blocks AI from being develloped for the greater good , i see no problem from having a doctor that is more capable in the field of medecine , AI needs alot of training to reach such stage but humans (goverments) are blocking the way already.
My stress test was great..but have build up in 6 arteries. God willing I can make it into my 80s.

What if you do not have high cholesterol? Do you need to have this test? I have an A1C of 5.7 and my doctor wants me to take a statin or get this test. I opted for the test because I don’t want to take a statin. What are your thought about this if a patient doesn’t have high cholesterol

Just as many people with low cholesterol as high have atherosclerosis and heart attacks. Stop making cholesterol the villain. Cholesterol is essential to good health and quality of life. Look instead at fasting blood sugar and triglycerides. Both should be under 85. You are pretty young - still being taught incorrect info in med school 😮
@@judyculang3766 That is incorrect. Individuals with higher cholesterol have a higher long-term risk of heart attack. It doesn’t mean that you will have one but it does mean you have an intrinsically higher risk. Additionally, the triglyceride goal is less than 150mg/dL fasting or less than 175mg/dL non-fasting. Certainly, I agree that controlling diabetes can help improve your cardiovascular risk. I also partially agree that cholesterol is an imperfect bio marker but it is one of the tools we try to use to help reduce our patient’s risk of heart disease.

Thank you for this video. Should the individual vessel scores, LM, LAD, LCX, RCA, and Other Agatston scores, add up to the Total Agatston score? I ask because mine do not, and I want to know if that is an error. Want to know if there was a calculation error, or if there is something more to calculating the total score than those individual scores.
Agatston score is completely separate and nothing to do with the actual calcifications. Total individual arteries should add up to total calcium score
My husband just had ablation surgery last week for A-fib. He was diagnosed with A-fib and heart failure just under two years ago. All the cardiologists he saw in the last two years put him on beta blockers and other meds to control it. Since the procedure the A-fib is gone but he’s still very winded. The night after the surgery he had a heart attack. My husband has never had high blood pressure, high cholesterol, or clogged arteries. This was a sudden thing when it hit. Two years before this issue hit him he had Lyme disease confirmed with bloodwork. Every GP,nurse,cardiologist, or any health care worker I asked about Lyme-carditis just looked at me like I was nuts. Not one of them had even heard of it. We are located in B.C. Canada and the healthcare system made him wait two full years until the meds began to fail him. He never took the experimental-RNA injection nor did he ever have Covid. What do you suggest we do at this point? Should he go on antibiotics to kill off the bacteria again? Please answer.
Definitely helpful for making personal decisions on choosing a specialty, however, when interviewing for a residency application, you cannot say that "because I don't enjoy anything else", right?🤣
@@user-cw8wr5mm5y ha most definitely. I think I recall on interviews saying something about enjoying the basics of medicine, being able to have the combination of inpatient acute care, and being able to be someone’s primary doctor in the outpatient setting

No amount of alcohol consumption is safe. Cardiovascular disease goes down, cancer rate goes in the opposite direction.
Is it dangerous for someone who has hypertension (under medication) to move to a high elevation place, like Denver, Colorado, or Flagstaff, Arizona?
I don’t see why it would be. Certainly can check your BP before/after the move to see if elevation made a clinically significant impact on your BP and if it sustained or resolved over time
@@MarcKatzMD From the American College of Cardiology: "High Altitude Exposure Among People With Cardiovascular Conditions" The article lists 8 areas of concern at high altitude, although their measure is at about 8,200 feet above sea level. I wasn't sure if that meant there is no concern at higher elevations below that mark, or if any impact would be negligible, even under physical exercise conditions, in general.
I had long term lyme, and did discover it when get a ACA on mine left lower leg, who was not directly seen by a doctor. I have done tetst and had a antibiotic course for a month where the ACA did fade a way, this is a year ago now, I do not now if I had long term lyme or not, I do not now if a ACA spot on a leg can also occur with short time infection or is there in stead of a normal bulls eye, I did see on pictures earlyer like 2019 I had no aca spot. I have now heart trouble, I have ectopics, more specific I had tests in hospital, like a echo, stresstest, 2 day holter, where nothing was seen, it is benign, but this do not mean that it do feel fine, sometimes I feel the heart jumping and comlete off, and feel in the body that bloodflow is altered, but have not yet get a blackout or dizzy. I comke to mine question now, is it possible the lyme is still there, because of pain in the feets, and silly feeling in hands or body, I had by the way big stress period, and this was a big problem making mine live miserable, caused by a nabure. I have naver experience a heart block like you tell here, except maybe the first grade who is with ectopics but on the ecg there is no block, machine says I have a sinusaritmie, special a supraventrical extrasystoles with abarancy conduction. I ask myself it lyme can also cause this or so, I had a dermatitis borellia, a version of it who cause skin problems. I can afcourse do a test with antibiotics for 2 weeks and see what happens.
