    It seems that surgery is not a solution for a biological problem. It only makes it worst, as long as we don't use medication to supress the inflamation.

    Regarding the lenghtening of the patella tendon he is completely right. I have done it and the AF and the position of my kneecap got much worst.

    I have had and still have arthrofibious in three different joints. Presently severe in a knee joint replacement. I would take the medication forever if it took away the acute pain I have 24/7.

    Why does knee capsulitis not 'thaw' after 2 years as frozen shoulder does? I suffered bilateral frozen shoulder 10 years ago, which gradually thawed and resolved over the course of 2 years. In 2018, I developed adhesive capsulitis in the knee with extension contracture after knee surgery. If they're the same thing physically, why does knee capsulitis not resolve on its own over 2 years n the way that frozen shoulder does?

      There isn't much known about why arthrofibrosis is more likely to resolve in shoulders than knees. Some researchers have suggested that there are different tissue-resident immune cells (macrophages) in the two joints that affect the outcome. It might also be due to the Hoffa's fat pad (IFP) in knees that becomes inflamed and swollen and gets pinched between the bones of the knee during activity, and even when standing. The IFP is full of immune cells, nerves and other cells that create more inflammation and fibrosis, and it can feel like a finger continually being slammed in a door when walking up steps etc. Because it's not easy to rest knees during daily life, the IFP continues to be injured and sets up a viscous feedback. The IFP is frequently cut during knee surgeries so this sets up inflammation from the start.

    Is there a possibility that arthrofibrosis will remain even if patient is having PT?

      Yes, good PT (gentle CPM and gradual weight bearing) can only help a joint recover, it's never a guarantee of healthy healing and some people still have arthrofibrosis despite good care. Exercising the affected limb can worsen arthrofibrosis.

    Almost two years suffering. Had a revision surgery three months after my first TKR (right knee). Contemplating another surgery to “clean up the tissue”.’ Have had all different approaches in PT, as you mentioned, the one not causing “insult” to the knee to try to get better. ROM has been maintained at 90 degrees pushing it. I do therapeutic massages. Do you think they help? My life has been turned upside down. 61 year old female, small framed, in shape. Now my life is totally restricted, pain, heat in the area… I’m under paychiatrist care, reumathologist care. I feel at the end of the rope. Did you recommend a CPM to wake up after a “clean up”’surgery? I had been told that it doesn’t work. So confused.

      I am almost the same. 3 years and 7 surgeries. 4 open surgeries due to infection then revision by new surgeon. Quad tendon repair caused me to be in a cast for 3 mths and that caused Arthrofibrosis. I’ve had 3 MUAs and still only at 70°. I also had a CPM machine and it was useless. I’m also petite (128 lbs) and now 64 yrs old. My life has been on hold for 3 yrs since it’s my right knee and I was a realtor but can’t drive. All the best to you.

      @@deniseblake2907 I’m so truly sorry…! Don’t know what to say. Nothing that I’m told helps me. Your infection complications must have been just too much. It’s so hard to see the light. Yes, lives on hold it’s what it feels. Arthrofibrosis is still active. Sending best vibes your way and hoping for a better outcome for both if us.

    Interesting that he is based in Glasgow. His colleagues from Royal Glasgow Infirmary rejected me as a new patient because I have arthrofibrosis, perhaps he could educate the sarcoma team about AF.

    Thank you so very much for this interview. I have suffered with this problem for 5 years after several knee surgeries .Two where total knee replacements. No one in the area where I live seems to know how to treat my knee issue. after much research on my own I found the description of me condition.. I am desperately seeking treatment. It has become difficult to even get a doctor to see me let alone treat me . I live in Myrtle Beach, SC If you have any information where to go to get treatment here I would TRULY APPRECIATE!! The pain is so debilitating and my life if always on hold..
