I’ve had both types of pneumothorax. My first was a tension one and I had needle decompression before a chest drain was placed. I was very sick couldn’t breathe couldn’t move to even ring the nurse. I was in hospital as they thought the chest pain to begin was a blood clot on the lung. However the next morning I had a tension pneumothorax. The treatment was terrible and the pain when having a drain put in. I needed 2 eventually and suction to heal that and it took 2 weeks to heal. My smaller secondary spontaneous ones have not needed draining many times. I’ve had 11 in total. The shoulder pain and shoulder blade pain and also tachycardia were with the smaller ones the pain was extreme for larger ones. Some paramedics didn’t realise my reduced breath sounds were in fact smaller collapses and found later on ct scans. I currently have a small pnumothorax and am recovering at home it began 23-24 December 2024 I’m resting completely as still in pain and still getting out of breath when I exert myself. My last was in may this year which took 8 weeks to heal as it was a lower one this one’s at the very top. I’m praying it doesn’t get worse as those drains are horrendous to be given while awake.

what about a slightly wide QRS(via Kardia mobileapp.) ? normal sinus rythme with wide QRS, good BP. good O2 saturation heart rate of 70 bpm...always 110/70 pressure. 59 year old male (former experienced Emergency medical tech.)

I was in the ICU intensive care unit in UK when I had a tension pneumothorax/Hemothorax if I got that right, which I had to have a chest drain, both sides.
I am so sorry to hear that happened to you. Thank you for offering your experience to help others learn! The hospital treatment for a pneumothorax and/or hemothorax would be a chest tube. That device would help remove the air/blood from the chest cavity and allow the lung room to inflate again. I hope you are well now!!

One note from here in the future (2024) is that suctioning the newborn's nose/mouth is no longer recommended unless "obvious obstruction to breathing exists" as it can lead to bradycardia
This was a Trauma, right???? ….The entire scenario…no one felt the need to expose the chest for injuries that may compromise ventilation??? ….There “appears” to be = chest rise and fall….that doesn’t sound definite to me….Better yet we’ll just cover up the chest……….Come on !!!!! You get a point for identifying injuries that compromise ventilation…No rapid trauma assessment either (DCAP!!!!) …might as well just call a taxi….I’d give you a fail….cool….

Thanks for the video it was very helpful although I was told when giving epi hold you’re supposed to hold the needle in for 10 seconds rather than 30 in my emt b class which one is correct?
10 seconds is likely long enough…. But since adrenaline tends to alter our perception of time, I teach 30 seconds just to be in the safe side.

wouldn’t the bvm push the food further down the airways when giving a breathe?
You are correct! However, if the abdominal thrusts and chest compressions don’t dislodge the object, then may be pushing it down into one lung may be the only/best option. Either way, you have on open airway.

Question: I noticed you did not mention active ulcers as a contraindication of aspirin. Why so?
Ryan, to be honest there are multiple medical conditions that could be a consideration when it comes to administering any medication. This video was not about the plethora of outlying conditions… it was specifically to educate providers on the key points of Chest Pain. Aspirin is a key player in reducing mortality and death from ACS, so I think one should evaluate the benefits vs. the risks with an ulcer that may or may not be an issue. Thank you for your insight.

You’re incorrect. The appendix is not in the right upper quadrant. It is the right lower quadrant.
