Inadvertently fucked with a paladin once. Party had the lich on his last legs and he just started laughing and laughing and laughing, the party was pretty high level and it was a slog but 4 level 18 characters. Anyway, my lich was very fire magic focused and he said to the paladin "May you taste my heart of fire, and may you choke on it" (Stole it from a tv show I loved as a kid) They killed the lich and set about looking for the phylactery. Paladin headed down into the study and he found rows and rows of stone statues. Party eventually discovered that they were petrified people, some were missing nobles, who hadn't been seen for decades, some random travelers, some adventurers, even some criminals. Among them they found something odd, a table with a single large red ruby. Several hours of checking for traps and magical bs later, the party uncovers it's an imprisonment spell. With a bit of investigation they notice inside is a small girl, she's still and not moving. They spend a night camping out in the dungeon. Next day dispel on the gem, out pops a petrified little girl, who couldn't be any older than 10 years. There is nothing strictly on the magic items list nor in the lich stat block that states a phylactery is inherently a magic item, nor does it say what it must look like, it could be any item technically. You can cast true polymorph on an item. The lich turned his phylactery into an innocent child, specifically just to give one big last middle finger, to whomever has to kill them. A big kicker was the paladin was an oath of the ancients so he was all about being kind to the meek and protecting the innocent. It was hard to rp it but she knew, what she was, to an extent, she understood that the longer she lived, the more likely "the bad man" would come back. Got to the point where she was blatantly begging for death, that there was no way out and she'd rather know that the people who finally gave her back her freedom would be safe. It was literally over in a flash, a big divine smite and the kid was gone, the residual arcane energy from the phylactery scorched the body down to little more than smoke and dust. He was kind of really pissed after that, debated about leaving the table, most of it was in rp but he did seem a bit miffed. We worked on it and he spent time re leveling as a barbarian zealot. He felt that he didn't live up to his oath and now all that was left in his heart was a deep religious hatred for undead. They did win the fight, I give them credit, but even adventurers can hit a breaking point, and sometimes a win can still be a loss.

A "nice guy" lich who made his phylactery a small part of a powerful royal regalia. He spends a great deal of effort to make regalia protect the wearer from all hostile magic and harm, even plane shift the wearer to secure demiplane designed to look a small bedroom, living room snd kitchen with well stocked larder that the royal family can use to defend against assassination attempts. It grabts energy and danafw rsaustance, poison immunity and immunity to necrotic and psychic damage in addition to the other benefits. The full set is an emchanted crown, pendant, scepter, orb and royal robes with the phylactery being a small gem with a gylph of warding set with teleport should it be removed from the crown that teleports the gem inside a box in airless cavern on the moon. Some point before the beginning of the game or even during it, the king of powerful real in your world acquired this this set as part of the liches machinations and they guard the regalia jealously because the make assassination attempts much more difficult. These would become national treasures, the last place anyone would look for the lich's phylactery, abd even jf the players figured it out, getting it away from the king is gonna be difficult as the court wizard, while mortal should still be almost as good as lich in terms of layering magical and physical protections, and probably has a bigger budget to work with. Its also possible lich is the royal mage or one of the castle staff using magic jar to take over said staff member's body.
