The Michigan Collaborative for Type 2 Diabetes (MCT2D) seeks to engage and empower medical providers across the state to accelerated dissemination and implementation of evidence-based strategies to prevent or slow disease progression of Type 2 Diabetes.

Rather than simply focusing on optimizing HbA1c as the disease progresses over a patient’s lifetime, the goal will be to support providers to deliver high quality behavioral and medical treatment to prevent, slow or reverse the course of the disease.
