Hello Allan, if you've been keeping up with medical science in the past few decades but people have moved past calling gender dysphoria & being transgender a "mental illness", because it's simply inaccurate. It's not a matter of having delusions, like psychosis, which is a mental health condition - it's a matter of genetics & hormonal abnormalities due to these mutations. As a matter of fact, if you're diagnosed with a mental health condition you actually can't recieve any form of transgender healthcare, because you have to be considered to be of sound mind before you can be diagnosed & prescribed. Transgender people can have mental health conditions & it's not uncommon for us to have suffered with say depression, anxiety, etc. but these aren't typical facets of the transgender experience that every transgender person goes through. These conditions are brought about through trauma, like bullying, abuse, etc. & these are only exacerbated by transphobic harrassment, bullying, etc. based on misinformation or incorrect assumptions such as "Transgender people are delusional". The truth of the matter is that it's much like being intersex & this actually metes out in the data. Intersex people often very obviously are intersex, they have a distinct physiological difference, like ambiguous genitals (which are often "corrected" [& that's a whole other sad kettle of fish] with surgery when they're only babies - giving them no time to actually determine their own gender identity), or not undergoing puberty in a clear manner for their assumed gender (ie. an intersex woman might not develop breasts, etc). A not-so-fun fact, 40% of men who go to a fertility clinic because they can't concieve find out they're infertile because they're intersex. Returning to trans people, transgender women (since I know more about them than transgender men - apologies) for example have repeats in genes involved with masculinising a fetus, to keep it simple, at some point during the hormone surges a fetus is exposed to (helping to shape the fetus' physiology - the genitals are built 6 weeks before the brain, fun fact) these hormone surges can stop pumping out testosterone as normal & you become "default", which is female. So you've been given enough testosterone to develop a penis & testes, but not enough for everything else, it could be an androgen insensitivity or a number of other conditions (& transgender women commonly present with androgen insensitivity). Depending at what point your masculinisation stops due to these mutations I mentioned prior in this comment, you essentially end up with a physiologically female brain. Now, don't buy into "Oh, men & women have different brains & one must be better", that's nonsense, but researches have autopsied the brains of transgender women who've been on estrogen therapy, trans women who haven't but maintained an identification as a woman until their death, cisgender women, a control group of cisgender men & cisgender men who had to take estrogen due to certain cancers of the genitals & found that transgender women's brains - both those on hormone therapy & those without - have structures most similar to cisgender women's brains (ruclips.net/video/8QScpDGqwsQ/видео.html), the men who'd been on estrogen therapy didn't have these same structures. We also know there's a genetic component because transgender parents are slightly more likely to have transgender children & twins are slightly more likely to both be transgender, suggesting these genetic repeats in genes involved with masculinisation/feminisation, are passed down. It's also not rare that a parent's child comes out as transgender & then the parent also comes out. Because of the stigma that has followed transgender people for a hundred or so years (we were reaching a point of acceptance across Europe - acceptance that we had in a lot of other countries mind, prior to European colonisation - prior to the Nazi occupation & their destruction of the Institute of Sexology which burned research into transgender people), many people, myself included, just didn't have the information to understand gender dysphoria. We simply went through our lives thinking everyone else thought like us & they werer "better at hiding it". Many of us learned to mask how we felt inside because expressing the characteristics of the gender we truly felt got us bullied. We absorb transphobic narratives that are all around us & we internalise that & can almost shame ourselves into never transitioning. You can't destroy the truth though, you can't forever supress that you feel uncomfortable in your gender. As a transgender woman who found out about the existence of transgender people at 22 & then found out what gender dysphoria entailed at 29 & said, "Hang on, other people don't feel all of these things? You're telling me men don't walk around wishing they were women & feeling no connection to masculinity?", it was certainly a trip - I wish I could've been told in school that what I was feeling was an understood thing & there was care I could get. I'd been feeling it since 6 years old, as soon as I found out boys & girls had different bodies & that I was going to grow into a body I didn't want, I'd dreamed of being a girl - as a 6 year old. By the time I found out trans people existed, I'd been so soaked in the "You've just got to man up!" narrative, that I continued trying to be that way until it made me so unhappy I had to seek therapy & I'm one of the lucky ones. Some transgender people don't have the resources, some are ignorant they're transgender until they die, some are in much worse situations & the abuse piled on top of the shame kills them, etc. The state of transgender healthcare across the world is also bad, you've got conservatives in many countries spreading misinformation that transgender children can recieve hormones & surgeries, meanwhile people can't recieve hormones until 18, puberty blockers are banned in many places & even if they aren't the waiting lists for a child are so long the vast vast majority of transgender people never have access to puberty blockers. In the UK when the ban was enacted, there was less than 100 children on puberty blockers. The waitlists for adults are even longer, just to be seen to for a first diagnostic appointment for gender dysphoria - of which you need two - takes 6 years. You need to be on a stable hormone regimen (& it can take months to reach stability & find a regimen that works for you) for two years to be considered for genital surgery & the waiting lists for the surgery are years long. This panic about something that isn't happening is accelerating care bans for all of us & that's not to mention that conservative governments are using people like Hillary Cass - a pediatritian with no training in trans healthcare & who explicitly met with anti-trans groups & helped draft anti-trans legislation for American republicans - to make this happen, it's not uncommon to see a government wave around a review explicitly done & sometimes funded by an explicitly anti-trans group (some of which posture as medical institutions by closely copying names of genuine medical institutions). Then there's the moral panic, the same exact moral panic that happened to lesbians & to gay men decades ago & wouldn't you know it, lesbians & gay men didn't come after you in bathrooms, didn't seduce your children to be gay in school, etc. they're just like everyone else. Transgender people are at far more risk of being abused than being an abuser (don't I just love having a 6x higher murder rate than cis women) & this is largely true of all LGBTQ+ people. *sigh* Well, I hope this depressing information has been helpful somewhat.
