    How about UPCR?Doctors are frequently forgetting to measure UPCR and some how waiting for the creatinine to fall.Cystatin C needs to be measured as well.

    Thank you for these videos!!!! There are so few people (specialists included), who know or speak about enthesitis. I was diagnosed this year in my iliac crest. For 2 years I was convinced I had a sports hernia and had debilitating pain. I'm getting shock wave therapy this spring. Fingers crossed!

    Interestingly, after a procedurevI had to stop my anti TNF, and I think the snowy weather has helped me not to flare so far. 2 weeks.

    Hi Jack. I am 52 year old female in Australia. I have been diagnosed with RA June 2023. I have had dropping platelets from the following year June 2024. From say 110 down each month by 10 to 15, till it reached all time low of 9 end od Dec. Bone byopsy has confirmed diagnosis as AATP. Amegacaryocytes absent. Happy to provide more pertinant info for your thoughts. AATP rarely encountered. Discussion and consideration of my case welcomed. Rare disease is extremely isolating and worrying.thks

    I have patients that come back years later after deciding to go the “ natural route” with deformities and erosions. I tell them ( and have always told them) right up front that RA is NOT natural, and it is fine to take the supplements but that it will not be enough, and they are taking the risk that their hands will become dysfunctional, even to the point where they cannot care for themselves . I offer to work with them to find a med that they tolerate and is effective, but make it very clear that I am not doing my job if I don’t give them this information. I don’t think we can possibly be too blunt.

    As a person with vasculitis I had to laugh when she said a CPR of 20 was VERY high. That's my normal right now but at my worst was 87.

    why'd you remove the MMF? my understanding is that patients with ILD should typically be on MMF for an extended period for maintenance. what was the time scale in question?

    ILD, joint involvement, have been reported comorbid in sjogren's and she is SsA posituve with dry mouth . Mycophenolate has caused some cases of brain lymphoma due to t-cell inhibition causing EBV proliferation, in a susceptible population, and of course we kniw EBV is implicated in Sjogren's. Rituximab could address both the Sjögren's and potential b-cell lymphomas (that represent a risk to sjiorens patients) by targeting cd20. If it doesnt work, possibky add belimumab since rituximab can raise BAFF Many sjogren's patients have also reported good results with hydroxychloroquine for joint pain

    Very interesting, thank you Did treating the virus have an impact on the autoimmune presentation and symptoms? One case that comes to mind was an HIV patient who developed a sjogrens-like syndrome and was treated with ganciclovir which i believe eliminated the symptoms. This could possibly work in some cases of potentially EBV-linked autoimmune diseases such as sjogren's, lupus, rheumatoid arthritis , etc.or undiagnosed conditions with similar features. EBV-specidic cytotoxic t-cells (autologous or allogeneic) may be another option in those cases and been studied by a few researchers, including Dr. Rajiv Khanna in Australia

    I have Luous & Sjogrens. I did the anti-inflammatory diet and it didn't help- in fact, my bmi got too low. The only thing that helped was hcq.

    10 years of symptoms & All the testing and protocols for every condition even RA but the ANA was OK - until after many complaints that we are missing something my NEW GP suggested I may have MCTD - so off to a RA , who was impressed but still uncertain - 4 months later - Diagnosed with MCTD as per a anti U1 RNP test. Now treatment is helping but very mixed results - some weeks are good many are bad Flares are horrible and debilitating - medications are expensive - until the 2025 changes for medicare - so Im waiting for my Enbrel. Good Luck everyone - please except the condition - for me it posted to SP many other doctor visits that went unexplained - even a surgery.

    Thank you for this info. I've been newly diagnosed by my doc, but I was on levofloxacin for pneumonia. The side effects are similar, but she is unmoving on the diagnosis. I am to see a rheumatologist. So far the symptoms are not bad and seem to be getting better without the Prednisone. This video has given me a better perspective of the disease and questions I can ask this rheumatologist.

    A therapeutic vaccine to raise endogenous antibodies for blocking p19 would be better. Just less profitable. A research group in a less profiteering part of the world will develop this eventually.

    Hi I have ploymyalgia rheumatica my Dr has had an email from a Rheumatologist saying that they looked at my medically records they thinking I don't have polymyalgia rheumatica, I have pain in only one side of my hips just wondering if you just mite have any thoughts on it please, been on steroids for 2 years now I'm on 11mg a day know my Dr wants me.of as soon as possible ,thank you for a great video, yours Peter peric from the UK 🇬🇧

    I was diagnosed with APS AFTER habing covid and started having major blood clotting issues..all about 6 months to a yr after haviing mild covid (i alsomwinder if the vaccination has something about it). Also damage to my blood vessels in my legs requiring medical intervention and PE. My health completely changed after covid.

    I've read that Arthrocentesis treats pseudogout by removing not just excess fluid but also removes the calcium crystals that cause inflammation. Is that correct? My mom had a steroid injection in the knee, but it didn't help and her knee is still swollen.

    Ketogenic diets drop ESR and CRP very similar to GLP-1 agonist. What does insulin resistance do to Treg, NKT, B cell and macrophage function? NIH study showed that ketogenic diet increases NKT and Treg activity and decreases B cell activity. So while your waiting for GLP-1 agonists, cut all the carbs and target nutritional ketosis. It's magic in rheumatology patients, especially SEA patients. J Dowd
