I think it was a delay/echo circuit that he's manipulating in real-time. You can get weird pitch-bends. He's also doing some things with his guitar at the same time.

Trust me I know a hell of a lot more about Aerosmith than most assholes who upload videos on here and don't know the difference between Sweet E and Pink. Come on now. BIIIIG difference.
