You can do the same as what I did in this video to raise your concern or you can go to the nearest PRC office in your area. Make sure to bring your valid i.d and birth certificate that show your birthplace.

Hello, nag reply na ba sila sa inyo? Ang kulitin mo sila. Nangyari po sa akin yan. Birthdate ang problema ko peru ang inayos nila password ko. Sabi ko na birthdate ang problema sa akin hindi po password. So ayun binago nila peru mali pa rin. So kinulit ko na naman sila hanggang naging tama.
