@7:00 - Seriously, I'm a big fan of your work but Simplify hasn't been in the market long enough to confirm superiority over ACDF in the LONG term eg 10 years. It's still classified as a new disc and barely commercially used for 4 years. This is important because when the M6 devices came out. You had surgeons jumping onto them and telling people prodisc is crap due to it's 3 piece semi constrained nature degrading the facets overtime and M6 is superior being made out viscoelastic technology. Yet it took 8 years to realise oh shit it's burning disaster. Yet the only disc so far that's stood the test of time is Prodisc after so many years even though it's a 3 piece mechanical device consisting of older technology... I also hope the study compared patients with similar injuries who had ACDF to Simplify ADR because generally speaking ACDF is used to treat with much more complicated cases in comparising to ADR eg oesteoporosis, facet arthritis, fractures, etc. This is another reason why in ADR you have such a small percentage of patients who are consider good ADR candidates vs ACDF which can cover a much wider range of ADR Contraindications. So naturally ADR may have better results because the patient injuries are less severe in the ADR candidates who may be fit young and healthy vs my grandma who's older with weaker bones, more facet degeneration etc and hence she may even take longer to recover

Sounds like a helpful innovation moving forward as the plug seems a likely breakthrough in connective tissue repair. Is the new cartiliage then formed as hyaline or fibrous? This could work well in conjunction synovial fluid injections for further pain management during recovery. From experience here in Westerm Australia, all of the general practitioners and specialists that I've seen or spoken with do not offer me as a public patient with many alternatives to non-athlete patients suffering with joint issues or to be precise cartiliage damage. The GPs simply do not care. The problem is not having much needed medical tech or truthfulness available to the general community as a whole, and only a select few have access. I realised that I am not an economically viable patient - meaning that I dont offer much that will help them in their own cause because I won't accept prescription medicines. People forget that there are other specialists that can assist when the general practitioner shuts the door. However, its an expensive process that takes time and lots of effort I have encountered, via academic research and direct engagement with allied health professionals much needed info to understand how the health system has become disjointed. Also we live in modern times yet we are offered outdated, archaic joint surgeries. These are made available to public folk here in Oz. That is 1.invasive joint replacement 2. invasive joint fusion 3. Arthroscopy for those with private hospital health cover. Other treatments are available but many older people are not equipped with the knowledge to research further because sadly most of them are on multiple concoctions of 'legal' drugs. My basic research has shown me that there is a lack of duty in patient care. It has also become obvious that big pharma knowingly adds poisons (such as i.e., titanium dioxide which is not fit for human consumption) to public medicines. It is quite astonishing really especially when you work out the economic benefits of the allied health aligned with pharmaceutical industries. The landscape of public health has got me questioning the progressive relationships between pharmaceutical companies, pharmacists and allied health professionals as it relates direcrly to patient care & outcomes. My opinion, if i may, is that an abusive pharma and medical system is designed to keep people sick within the hamster cycle. It will take a behemoth and for society to wake up in order to shut big pharma down, return to farm food, and authenticity of the professions that health care workers can apply necessary strategies providing patients with real, humane care. Therefore creating a more harmonious world for all. Until that point we are trapped in a vicious cycle that is geared towards dysfunction and as a result we move towards an unbalanced dystopian future. It is my hope that the plug continues to innovate and is given the opportunity to be made available to a wider audience; individuals via moral streams, and not through companies that make such innovations available only to the few. But then again, its not up to me. What will be, will be. I extend my best 😊

what are thoughts about the TOP’s system vs typical fusion for double pars defect?

Presently, I am in consideration for the 3Spine Motus surgery now. Next week, 11, 29, 2023, I will travel to see a Doc in NC for evaluation. The evaluation will also include an MRI, Cat scan and bone density test. My pain is primarily at L4,5 as explained by other Docs who have suggested I have a fusion. My concern with a fusion is the high percentage of people who have adjacent vertebra failure/disease later and have to go back for more surgery or just hurt for the duration. I am not a medical person but as an engineer, I have spent countless hours learning about the spine, surgical procedures and outcomes over the last decade and half. So far from my lay medical perspective but as an engineer I believe the Motus device makes a lot of sense technically in a number of ways. The fact that one retains some flexibility alone is very encouraging. I also like the fact that the Motus device is anchored to prevent device slippage is also most logical. I have read too much about artificial disc slipping and the person with this device is in horrible pain, again, and has to be reopened for more surgery. From my reading it seems the device has been designed with excellent materials for longevity. This discussion panel from Docs who have actually done the procedure and have supporting data on the outcomes has also encouraged me considerably. I have not been able to find any Motus reviews from people who have had this device installed so far; perhaps they are all out very busy living life again ! Personally, I believe the Motus device will, HOPEFULLY, be a very common solution for many of us who have been beyond miserable for years, and in my case, 20 years. I am not expecting to ever be pain free but if the pain level can be turned down enough where I can enjoy taking a walk or going on a vacation with my sweet wife without being miserable, I'd be VERY thankful !!!

I am a 37 y/o male who has undergone two surgical interventions for pars defect in L5 with spondylolisthesis grade I (L5-S1). The first one was a pars repair with the wire but, 6 years later one of the screws failed and had to go in for surgery again. It was a similar intervention but instead the surgeon used hooks and screws. Even though my back felt more stable since day one, I still had some discomfort (instability sensation) and back pain after playing soccer or running for more than 30 mins. It actually never felt 100% perfect, and honestly it has been extremely frustrating since I have always been a high performance athlete. My question is: from this new device why not to place the upper prosthesis a little further in so that the anterolisthesis can be retracted a little to a lower grade? Also, would I be a candidate for this type of surgery? Best regards, Jimmy.

2 Questions #1 can a revision L4/L5 that did not fuse be done by just removing and adding a new cage or bone graft without touching the screws and rods if they look pretty solid? #2 if a surgeon did not remove all or enough of the disc when he did the fusion could that be the cause of pain?

This is the man who saved the life of the lone survivor of the terrible Nick Adenhart fatal accident.
