2HEARTS provides a safe, supportive environment where kids get to meet others who have experienced similar frustrations and fears. One of the main missions of 2HEARTS is to educate and inspire. These kids need to learn how to take on the responsibility of their own health care and become independent adults who can carefully maneuver the gauntlets of life with a chronic illness.
2HEARTS has just published their first workbook entitled The Road to Transition. This book is an educational tool that will introduce these kids to being involved in their own health care plan. Transition is a long process and should start as soon as 11- 12-years-old. “It's not as simple as getting the name of a new doctor and going to see them when a patient turns 18.” according to Dr. Craig Sable of George Washington University.
