As the DM, you should always be aware of what your party is capable of rolling on the high end, and NEVER put them against something they have no hope of beating. If a 15 or higher doesnt hit, they shouldnt be facing it. Only exception is storywise, when the DM doesnt plan to kill or at least perma kill a character, but only wishes to give a tough fight, or to handle a murderhobo.
