    FYI it's A LOT easier to have your assistant put the gel on the probe prior to putting the cover on it, just have them hand it to you and you receive it with your hand in the probe cover.

    Indwelling catheters are major sources of in-patient bacteremia. The skin is never rendered sterile. I strongly urge the placement of a small amount of Betadine ointment at the puncture site before applying the dressing. I change dressings daily. Having inserted hundreds of central lines, all without complication, I can say that enhancing pt. safety is always rewarded.

    What's the reason for not touching above the yellow line of the sterile gown? Also, why shouldn't we raise or lower our arms?

      All of these proceedures and rules are to avoid a central line infection. Using the same experience learned form surgical infections.

      There are germs everywhere, including the air, on hospital surfaces like tables and bed rails- even with disinfection. You are not allowed to raise your hands above your head or out as that increases the amount of germs passed on to your sterile gown and gloves by the air. You aren’t allowed to extend your hands pass the cuffs of the gown without gloves because again- this can cause germs to spread onto sterile surfaces. The medical devices demonstrated in this video were sterile- meaning it was sterilized to completely- 100% kill all germs and microbes. Why these procedures? A center line infection can be catastrophic, causing death and pain to patients. A line placed through the body- into the heart must be sterile to avoid a central line infection. Even one germ could cause infection.

    Also, i have seen that some physicians wear two sets of gloves. One pair of larger and darker indicator gloves, and one pair of lighter outer gloves. That configuration shows very clearly if the outer pair of gloves are broken. I was just wondering why the glove configurations can very from hospital to hospital and what the benefit is for having one pair of gloves.

    Huh. I've seen hundreds of these done in the field with none of these fancy procedures. The Arrow kit is used, but apart from that, nothing special.

    The line doesn’t need to be flushed through with saline prior to insertion? Seems like an air embolus would be likely if not

    they need the sonogram for every iv i get, unless they go in my foot or ankle,, but they wont because of type 2 diabetes, but my primary dr takes blood from the top of my foot,,,

      Except, you don't need an assistant, don't typically need an ultrasound, and the entire procedure can be completed in about 5 minutes or less.

    I had one of these for plasmapheresis, its very useful and i perfer it over an iv for plasmapheresis

      Justina Branham i was given the option to be put under, which i chose to be put under for the placement. Glad i was but waking up and the first couple of days were pretty painful, they gave me alot of pain meds within like the first 12 hrs of me actually being awake, but after like 3 days with it it didnt hurt as much. But i was also allergic to the covering they used so we had to make sure anytime we changed the dressing it was the correct kind and stuff. Other than that it wasnt too awful, the only other thing i hated was having to sleep in different positions because i could lay on my sides and be comfy so i slept on my back which i hated

      Tyler Asher I also had one of these when I was diagnosed with transverse myelitis, also for plasmapherisis. Mine was done with no sedation or anesthetic other than lidocaine and it was one of the most painful procedures that I have ever experienced. I'm still traumatized.

      Because basically the wrappers of materials he's opening/handling are not sterile so it doesn't make any sense if he wear that sterile gloves.
