College Soccer Goalie: A Great Exercise to Build Hip Strength After a Hip Adductor Strain | Level 1
Watch this collegiate soccer goalie rehabbing from an adductor (groin) strain. He’s currently in phase 3 of his recovery. His exercise must progress so that he can return to the play.
In this video, he performs side-lying single-leg hip extension. His technique must involve full hip extension, maintaining a neutral spine, and preventing the knee from falling into a valgus position. When done correctly, this exercise improves gluteal strength, critical when recovering from a groin strain.

I had a shoulder dislocation plus fracture to humeral head. The dislocation has left me with Hill Sachs and Bankart lesions. As I'm 67 (and not embarking on anything sporty) I've been told no surgery. I still get a lot of pain and subacromial subdeltoid bursitis if I just do normal activities like cleaning my house. Having to pay for that now which is a bit irksome but needs must. Do you think working primarily on int and ext rotation as well as retractions and protractions is the best bet? I also twisted my elbow when I fell and hurt my wrist. Plus, probably the worst thing, is I have hypermobile shoulders. Guess it's why I dislocated as I was falling. Thanks for any advice. I've seen so many physios but the training in my country seems mediocre at best. Appreciate you've told others to get surgery but they won't contemplate that here in the UK. Thank you.
Hi. Sorry to hear about your injury. It's sounds like you are in quite a dilemma. Not, wanting to do sporting things, is very different that being able to push, pull, reach, and carry during your activities of daily living (ADLs). We hope you can find more compassionate health care providers. What they are failing to understand that you face an increased likelihood for a total shoulder arthroplasty (replacement) based on your injury. This surgery, and rehab, will cost more money, and will involve more health risks for you, when you need it 5-10 years from now. With regards to the best exercises, it is really individually dependent. There basics of restoring pain-free range of motion and shoulder girdle strength and stability must occur to improve your independence with ADLs. The exercises you mentioned should be fine to begin with, in addition to rowing and pulldown movements. There are other exercises to help with shoulder stability that involve quadruped positioning (your physio can help you with these). We would also encourage you to do exercises for the uninjured side. There is enough good research for "cross-education" for rehab. It means to continue working on the uninjured side, and there will be cross-over. Be patient and advocate for your health!
@@EvolvePhysicalTherapy Thank you for your considered reply. Very kind of you to take the time. I will look to adding some rowing and pull down movements and find out about those quadruped stability exercises. I've heard about "cross education" but have occasionally found I've managed to hurt the other arm. In some ways that's a good bit of learning as it indicates that exercise will also be aggravating the right injured arm. Mostly what I've learnt from these experiences is that as a hypermobile person I really can't do any strength exercises in an eccentric or arm elongated position. It's either isometric or concentric for me. My poor tendons just aren't up to many standard exercises. I appreciate that "cross education" is more about nerves and brain connections, but trying it has also helped my conscious brain consider and re-consider. None of the physios I've seen fully appreciate the hypermobility angle (and I'm not laying blame here as it's a new-ish thing not fully understood by anyone) and so I have to work it out for myself ...... which has added to the stress and recovery time as I learn from re-injuries. 😊
Hi. It might be in your best interest to find a PT who specializes in hypermobility, possibly Ehlers-Danlos Syndrome. These physios have extra skill and knowledge. There should be a national database that you can find an appropriate practitioner. Don't give up!

Years ago I looked for such a physio but at the time there was one unit in London (UK) but completely the other end of it from me ... a 2 hour trip one way when I was in considerable pain. I've just re-looked and there appear to be a lot more now that are centrally based and much easier for me to get to. I will be exploring various websites over the next few days. Thanks again.
How do you keep from accidentally getting a higher value due to your own pushing with the dynamometer? Does it also notice your own efforts?
Hi. Thanks for the question. There is a skill that's involved with the hand held dynamometer. The patient performs a "make test", which means they push or pull with maximum effort without the body moving. It's a full effort isometric. If the patient is too strong, bracing is used.

Hi. Thanks for the comment. No need to worry about him. It was a struggle at the beginning of his rehab, which is common after ACLR surgery. He did make a full recovery , and went back to playing football.
@ i literally stopped playing soccer because it was one of the worst pain I’ve felt and it kinda bummed me out and never went to get it checked out this is like almost 6 year injury

Hi. Thanks for your question. He is using a Neuromuscular Electrical Stimulator (NMES) on his quadriceps. This device helps him achieve a stronger quadriceps contraction and is very useful in early ACLR rehab.
do you know what type of graft was used?

I had tibia fracture surgery 25 days ago. After how many days/months I can take Physiotherapy?
Hi. Thank you for your question. Sorry to hear about your injury. We typically find that after a tibial fracture, people start physical therapy/physiotherapy on an average of 4 to 8 weeks following the injury. It depends on the severity of the injury and if surgery was involved. Best of luck with your recovery.

Ill be doing more of these. I fractured both tib and fib in august, its about 10weeks poat op orif, but my knees and ankle are still stiff, especially when going down stairs with the unaffected foot
Hi. Thanks for your comment and sorry to hear about your injury. You’ve got a long road of recovery ahead of you, but it sounds like you’ve got a plan. The knee and ankle stiffness will subside, and you should look for improvement every 2 to 4 weeks. Stay positive and you’ll achieve your goals.

What Is the risk of re injuring the acl in rehab? Since the exercises do involve the acl it can’t be 0 right?
Hi. Thank you for your question. Exercises must be performed to restore full function. Rehab is based on a criterion-based system and involves 4 phases. Exercises are focused on the stages of healing and length of time needed to restore strength, dynamic stability, and power. Risk will never be zero, rather it’s mitigated to maximize function and prevent the likelihood of re-injury.
Why he landing on his toes? Is that purposeful?
Hi. Thank you for your question. He is landing on his toes because it is the preferred way to land, in an athletic position. Landing this way will allow more dynamic counter movements, either laterally, forward, or backward. Learning how to land this way is critical for ACL rehab. One should never land flat footed.

My knee won’t fully extend after 4 months TPF
Hi. We are story to hear about the lack of knee extension. What manual (hands on) techniques and exercise exercises is your physiotherapist/physical therapist doing? As we are sure you know, it’s critical that you restore full knee extension.

There might be a need to address hamstring restriction. If one doesn’t achieve full knee extension after an injury/surgery, the hamstrings will shorten in length. Soft tissue mobilization of the hamstrings and mobilizations of the knee (tibiofemoral) joint might help.
In my right hand shoulder proximal humerus surgery done by ortho doctor and surgery done 6 weeks ago but till my hand frozen at grip and shoulder side I shall not properly raise my hand as same normal condition pl suggest me for the same with physiotherapy exersise and recovery time.. and when I will be drive whihicle and start my job ?
Hi. We are sorry to hear about your injury. Are you currently in physical therapy/physiotherapy? You will not be able to rehabilitate fully without one-on-one treatment. Initially, the therapy consists of hands-on (manual) treatment to restore range of motion (ROM). This is a very critical stage, and failure to have this treatment will impact your full recovery. Recovery time will be 4 to 12 months, depending on the fracture type and surgery. The rehabilitation team determines clearance for driving and returning to work, factors in hand dominance, and the kind of job(s) you perform. The exercise in the video is after many months of rehabilitation.

Hi. Sorry to hear about your hip pain. There are many reasons why your hips may be hurting. The first step is to get a detailed assessment and evaluation by a skilled physical therapist/physiotherapist. They will be able to determine not only the symptoms but the cause. Hi. We are a little confused by your question. He had surgery and this exercise must be performed to help with his rehabilitation. With ulnar collateral ligament reconstruction the rehab involves progressive loading to allow him to return to elite throwing.

Hi. He’s using the Keiser Functional Trainer which is pneumatic resistance. He’s also later in his rehab, allowing for increased tempo of movement.

Hi. The Shuttle Ultimate is a highly comfortable exercise machine. There is 3 inches (8 cm) of padding. This exercise, when performed on the Shuttle Ultimate is not uncomfortable.
I too am not able to do a straight leg raise, my foot wouldn't raise a cm above ground level ...I think this exercise should help ..I have undergone a fracture patella surgery
Hi. How long ago did you have your surgery? Are you using an NMES unit? ruclips.net/user/shortsP_SyZC5k0QY. This video shows the isometric quadriceps which must be mastered before the straight leg raise (SLR). Make sure you perform these exercises in tall sitting.

Yes. Talk to your physiotherapist/physical therapist about getting you a NMES unit. If used properly, NMES can make a significant difference!

Hello,how can I improve my uneven walk? My surgery done on late December last year.
Hi. Walking should be normalized within 3 months following surgery. Has your leg strength (compared to your other side) been measured? Are you still in physical therapy/physiotherapy? What is your knee range of motion (ROM)? What exercises are you still doing? What exercises have you done?
I have tibia+fibula fracture grade III, having implant. Strength is not as good as compared to the other one No I'm not in physiotherapy Knee motion is normal but issue in ankle movement.
@@AbhishekSahu-fi8wp Hi. Sounds like you should go back to physiotherapy. You will need to restore ankle ROM and improve your strength. Have them revisit the basic strengthening exercises, which are the foundation of your recover. More advanced exercises should not be performed before you have mastered the basics (restoring ROM and basic strength for the lower body muscle groups).
Got in car accident. Broke my tib fib.... had an orif surgery of the tib placing a titanium rod and screw in the bone and its 3weeks post op. Currently dealing with what seems to be swelling or a muscle hernia or a bit of fascia close to where the fracture occurred. I'm not sure what to do, waiting to hear what the docs say. Any info would be greatly appreciated
Hi. Sorry to hear about your injury. It is not uncommon to have considerable swelling following surgery involving ORIF. You should send pictures to the surgeon and their team. The first phase of rehab focuses on pain and swelling control. Were you giving compression socks/leggings? Are you elevating your leg? Are you icing your leg?

Hi. Thank you for your question. The exercise's purpose is to build her explosive leg power. This is not a beginner exercise, especially for rehabilitation of hamstring strain (tear).

Hi. You’ve asked a great question. You will be able to do bench press again. It may take 9 to 18 months before you feel comfortable with the movement. Also, you will need to have shoulder strength measured, objectively, using a handheld dynamometer, at approximately 4 to 5 months out from surgery. You will then need to have shoulder strength tested every 6 to 8 weeks to measure your progress. Make sure you measure both sides, the surgical and non-surgical side.

Doing a static isometric hold for your quadriceps, and holding it for five seconds is the best exercise to start with. There should be no pain when you perform the exercise. If you’re still having pain, it is likely that there is swelling, still underneath your knee and in the knee joint. Do you have access to a stationary bicycle?

I have had a a slap tear and bankart lesion for quiet some time I couple Months I had stem cell therapy treatment and it’s shown a significant difference would these exercises be okay for me to do or would you recommend something else
Hi and thank you for your question. You will need to start with the basics of strengthening and stabilization for the shoulder girdle before attempting to throw. You would be best served going to a well-skilled physical therapist who will guide you through the phases of rehab. Remember, the patient in this video is a former-collegiate athlete and was 8.5 months out from surgery.
These videos are much more helpful when someone is talking and narrating which is the injured arm and why they’re doing that… instead of blasting music
