The light bends as it passes through the anterior cornea considerably (refraction). Therefore the marginal rays passing through a, say 5/6 mm pupil will bend and may pass through a 3 to 4 mm pupil. The light does not pass through a straight line as it passes through cornea. Therefore a 1:1 relationship between cornea and pupil is not ideal. Just because the pts pupil is 3 mm in mesopic condition, does not mean that only rays pass through the 3 mm of cornea will only be relevant.

Great thoughts here. But when you talk about angle alpha, you are not just talking about decentration on xy axis. You are actually meaning tilt too. So if you are only taking of decentration, well, that’s acceptable. But in reality, eyes have dentration and tilt both. That is why we say angle. So, I would say angle alpha almost always should cause residual astigmatism. There is a study by dr koch on this concept

Hi, I thought the peak of the Gaussian like curves are showing resolution or the image quality in terms of vergence from eye and not refraction. Am I wrong in my understanding? I thought VOSTF is a good way to see the DOF on one hand and resolution on the other. Would be great if you could explain that.

Every single person considering refractive surgery should watch this. The current standard of care that ignores pupil size is criminal, not that even people with small pupils should get these surgeries when you consider the much worse complications like corneal neuropathic pain.

This looks so simple compared to a Vitrectomy.. why isn’t it widely offered ?

Had comfort lenses put in 4 years ago. Blended vision mix - near and middle vision one lens and middle and distant the other. No need for glasses. I had the angel wings effect for a few months but nothing for a very long time now. Still very happy with the choice the surgeon made.

Just a shame it's way too expensive

My ophthalmologist will only use Vivity for EDOF patients. He doesn't like Synergy or PanOptix diffractive lenses. Also he will not use Eyhance monfocal plus lenses. I guess it's Vivity for me. I hope it's because it's a better lens and not because it's the most expensive.

Beware of this eye butcher who destroyed a patient's eyes & life with lens AND laser ops which has made them suicidally depressed! I have seen reviews by other patients damaged by this surgeon who may well be a psychopath just like scores of others like him worldwide who probably sleep like babies despite the horrific suffering they inflict on people in their ruthless pursuit of profits!
