This is extremely interesting. But I think the macrophages are just turning the monosodium urate crystals back into water soluble uric acid, and then the uric acid will float around in the blood stream for awhile then redeposit in tissues as urate crystals again if the uric acid greater than 6.5mg/dl is not excreted. Please correct me if I am wrong.

Monosodium urate crystals forming in joints are engulfed by neutrophil leukocytes and damage the lysosomal membranes of these cells, thus causing cellular disruption. The generation of superoxide free radicals and release of lysosomal enzymes into the joint precipitates an acute inflammatory reaction. The release of interleukins (particularly IL-1β) and other inflammatory mediators from monocytes and tissue macrophages also provides an inflammatory stimulus

This is why it takes so long for a gout attack to pass. Depending on how much uric acid crystals have built up in a joint, your white blood cells need five to 10 days to consume them all. White blood cells (aka leukocytes) are made in your bone marrow, and they are found throughout your body.

The reduction of uric acid crystals would reduce pain. The build up of uric acid in the body is what leads to gout attacks. The stages of Gout Stage 1: Asymptomatic hyperuricemia: You have an abundance of uric acid in your blood (hyperuricemia), but don’t have any visible symptoms. Stage 2: Acute gout Buildups of uric acid (or crystals) start forming in a joint, which can lead to severe inflammation and pain. This can make your joint warm to the touch (a gout attack). Stage 3: Interval gout (intercritical) The symptomless stage between gout attacks. This stage can last for a few days or up to several months or years. Stage 4: Chronic tophaceous gout This is the stage where tophi develop in your joints and the tissues around them. They usually happen if you don’t treat your gout for a long time (about 10 years or more).

Everyone produces uric acid naturally, even people who never get gout can have uric acid crystals in joints with no symptoms...it is the job of the kidney to move the uric acid along...so if I develop chronically high levels of uric acid, and chronic gout, what are we supposed to be looking at re function of kidneys besides A1c test? I have Metabolic Syndrome and am not healthy. Dietary triggers are important to note but different sources say dietary is not cause, it is a trigger. I have been on ketogenic diet 85% for a year. I have chronic gout now and cannot tolerate gout medicines. I use prednisone which I hate too. My second covid shot has caused my knee to just blow up and swell with gout. I have been unable to bend my leg for 23 days in spite of full doses of prednisone. I reported this heightened adverse effect to the CDC Site on a data collection form where they are tracking vaccinated people. When the gout subsides, I will start ripping off the bandages and look to why this continues to happen.

No processed sugar, I don't have gout but I get foot cramps from sugar, so I eat fruit to replace the horrible sugars. And certain fish and meats is a no no.
