I think it's his Mom. Even a close friend doesn't result in that face and that sudden change of sides. She just looks like a maternal figure to him in that moment. So I believe that it's his mother.
The theme also sounds like a mother's lullaby to her child. She's also crying. See Ten's expression of hate/fury melt at the sight of her clinches it for me.

Mind-blowing, since it was always hinted that the Doctor came from Gallifreyan High Nobility. What better backstory than a wayward son running from his father's influence on their home planet?
