It never ceases to amaze me the amount of demos that fail to accomedate the opposite arch. 14:15 maxillary right buccal. Where's the patients jaw? Wheres all the extra room you assume I'm going to have?

Yes, the cavitron is an excellent tool. It is very helpful in so many ways: reduction in operator fatigue, lavage, and initial gross debridement prior to scaling and root planing.

@Gloria Galvis Hi Gloria! We are learning the sickle instruments now. Leaning how to find the right working end for each teeth area and wrist activation. The performance test will evaluate our Fulcrum, wrist activation, chair position, light. We also need to describe each instrument. If you have any video for sickle instruments, it will be wonderful. @@gloriagalvis1419 The Gracy 13/14 Universal Curet and may you show how the technique for hte linguals? In the anterior video it is shown but not in the posterior. I struggle so much with the posterior lingual both max and man. In our cassettes at school we have the Gracy 11/12, 13/14 and 1/2. YOUR VIDEOS ARE AWESOME and have helped me pass the more difficult competencies while in school. Oh how I wish you were still creating videos or had more. please create more please please with a cherry on top.
Hi gloria have you ever seen subgingival curettage without anesthesia ?
There are times when a patient may prefer to avoid local anesthesia for a variety of reasons, however unless treatment is local to one or two sites of pocketing that does not extend beyond 5mm, I would not recommend treating without proper pain management. Local anesthesia is an important part of pain control during a thorough and effective scaling and root planing procedure. In my experience, if the patient is not comfortable they will not tolerate treatment. Furthermore, as a clinician, I am unable to perform comfortably and to the best of my ability if I know my patient is in pain. Another fact to consider is that when utilizing an anesthetic with epinephrine (vasoconstrictor) there can be added benefits such as a decrease in bleeding at the site. That being said, there are other topical options for pain control, but in my experience there is nothing more effective than injected local anesthesia.

i thought the terminal shank should be parallel to the long axis, the second one looks like it’s correct why is that wrong ?
The lower shank will be parallel to the long axis of posterior teeth. However, when working on anterior teeth, the lower shank will seem to wrap around the tooth. The technique highlighted here may be reviewed in the text entitled "Fundamentals of Periodontal Instrumentation," by author Nield-Gehrig.

Hmmmm… the face is too close almost 0 degree when it goes to interproximal space!
Thank you for your comment, the angle is closer to the surface due to the limitation of space on the typodont, please be sure to maintain proper angulation so not to burnish calculus when performing patient care. Additionally, if you are unable too maintain proper adaptation, please consider another instrument for proper calculus removal!

What is meant by facial surface towards & away means?
"Towards" and "away" refers to the relationship between the tooth surface you are instrumenting and your position as the operator. For example, if you are a right-handed clinician and were instrumenting an anterior tooth surface "towards" you (surface closest to you), then you would seat yourself at an 8 o'clock position and adapt your instrument to the midline of say... tooth #26 and perform strokes oriented towards the distal aspect of the tooth. The anterior sickle scaler, as shown in this video, is of a simple shank design. Due to the simple shank design, this instrument is used for calculus removal in anterior sextants. Posterior sextants require a sickle scaler of complex shank design, you may view some details of this instrument here ruclips.net/video/JbVfIuelFBA/видео.html . While I have yet to post a video of this instrument in action, the posterior sickle is similar in shank design to the universal curet, you may view this video here ruclips.net/video/MWrFynWCSHo/видео.html . Keep in mind that one major difference is that the curet design is intended for subgingival instrumentation, while the sickle scaler is primarily for supragingival instrumentation. I hope this helps! The anterior sickle scaler, as shown in this video, is of a simple shank design. Due to the simple shank design, either end may be suitable for calculus removal in anterior sextants. This instrument may be paired (same design on either end) or non-paired (different designs on either end). If you have a non-paired anterior sickle, choosing the correct working end will be based upon the type of calculus present and accessibility. I hope this helps! Thank you for your question!
what does "col" mean?
Below the contact area between two teeth, the interdental gingiva assumes a concave form, or a valley-like depression, that connects the facial and lingual interdental papillae. This concavity is called the "col" and is the site where disease begins. There are some great images that may help you visualize this anatomical structure, simply google search the term "periodontal col." Hope this helps!

@@alesy8224 Before I answer your question, I would like to discuss the goal of probing. The goal is to comprehensively assess tissue attachment for the complete "circumference" of the tooth. With that said, we use a "bobbing" motion or we "walk" the probe in a manner that assesses depth all around the tooth. We record three measurement depths on the buccal aspect and three on the lingual. Each of these measurements represent the deepest depth noted in each of these six defined areas. Therefore, when assessing the distal area to determine the measurement of depth that is to be recorded, you must record the deepest depth noted when bobbing your probe the distance from the distal line angle to the col. We can apply this to the direct buccal measurement = the deepest measurement noted when walking the probe from distal line angle to mesial line angle. And, again we apply this to the medial reading = the deepest measurement noted from mesial line angle to the col. I hope you found this helpful! Thank you for your question!

For the maxillary posterior teeth, lingual surfaces away (15:04), it looks like your fulcrum is on the same tooth you are exploring. Aren't you not supposed to?
Great comment! As your standard guideline, we are taught to fulcrum 1-3 teeth away from the working area. However, remember that the concept of ergonomics is also important and maintaining a neutral hand and wrist position is critical during instrumentation. We are taught guidelines for optimal instrumentation which are to be used as just that -> a "guide" to our proper application of all our knowledge. Consider this.... if an arch is partially edentulous and there are no adjacent teeth to the tooth surface in need of debridement, where would you fulcrum?
I'm not a Hygienist but there's a sonic (Not Ultrasonic!) scaler for home used sold on Amazon.
