How would you use some of this terminology during OT documentation? Are OTs able to document that "the patient has agnosia" or, "the patient is showing signs of agnosia"? Or IS experiencing hallucinations or MAY be experiencing hallucinations? I guess my question is: what ultimately goes into diagnosing patients with the terminology used in this video? And what are OTs able to document and NOT able to document? (Also, I guess terms like delirium, dysthymia, etc. have specific diagnosis criteria so in that case diagnosing falls outside of our realm and we can document something like the patient may be exhibiting symptoms of these disorders or OTs can report their suspicions during team meetings) I apologize for the long winded question! Btw I love your videos and I learn a lot from them <3 Thank you so much!

As a patient, it is nice to see an OT that understands some of this. I have always gotten worse from exercise. Before I understood PEM, I just believed I didn't not have enough discipline, etc. And kept trying for many years, but it never worked. Anecdotally, I know many people from support groups that have gone from mild or moderate to a permanently severe or very severe state. I think it is helpful to keep in mind (as you seem to realize) that having ME/CFS is not the same as idiopathic chronic fatigue which can be from various causes. Post Exertional Malaise (PEM) is an essential part of the ME/CFS diagnosis now. There have been past criteria for CFS that did not require that and undoubtedly skewed results of various studies, etc. Another thing to mention is that hEDS/HSD (hypermobile connective tissue disorders) is a fairly common comorbidity with ME/CFS and PTs and OTs also need extra knowledge about treating those conditions as well.

Quick clarification of the Sign of Benediction (SOB). It signifies a more proximal median nerve injury. The SOB can be seen when asking the patient to make a fist. The median nerve innervates the FDS and the RADIAL 1/2 of the FDP. Damage results in an inability to make a fist, but some slight flexion of digits 4 and 5 is seen due to the intact ULNAR nerve (innervates the ulnar 1/2 of the FDP).
