Hi Tina - The KX modifier is to be reported in addition to the codes for insertion of pacemaker (33206, 33207, and 33208). Haugen Academy recommends consulting the local coverage article (LCA) addressing pacemakers for your jurisdiction. If your denials are not following the LCA, we recommend contacting your Medicare Administrative Contractor (MAC) to see if they have an unpublished policy update.

Hi Michelle, Here is the response from our education team. No, the excision for the LIMA harvesting for a free graft is not coded separately since the graft is obtained from the same operative site. Please reference guideline B3.9: “If an autograft is obtained from a different procedure site in order to complete the objective of the procedure, a separate procedure is coded, except when the seventh character qualifier value in the ICD-10-PCS table fully specifies the site from which the autograft was obtained.”
