Discussing 'Do Not Resuscitate' or DNACPR with Patients
Просмотров 3502 года назад
Today’s topic is something I tend to struggle in the beginning of my career, discussing DNACPR. I used to feel that I won’t be able to explain clearly to my patient or their family members and worry about getting misunderstood. So, I’ve made a short video on how to step-by-step go about explaining DNACPR. If you’re a junior doctor and plan to discuss about this for the first time, make sure you...

    • @priyameds
      @priyameds 3 месяца назад

      @superduper5949 the basics such as venepuncture, ABG/VBG, catheterisation, NG tube insertion skills will be helpful. advanced skills such as Ascitic tap/drain, lumbar puncture, chest drain insertion can be done as extra if you're placed in medicine specialty rotation like gastro, respiratory. But it's not required for FY2.

    why we DNR a patient who is specially suffering from chronic infection e.g pneumonia....plz if u know explain.

    • @priyameds
      @priyameds 4 месяца назад

      I wouldn't say chronic infection, the reason for putting a DNAR, is based on Patient's performance status, their comorbidities (severity of heart failure, CKD) - and I always think for myself unless if there is reversible causes 4H and 4T as per ALS guideline, I ensure to look at the patient from every perspective (functional Baseline, severity of comorbidities, quality of life, age and frailty score) before deciding on DNAR. Hope that helps

    • @priyameds

      This is really tricky. I have seen many patients where we struggle to determine clinically when they present with acute knee swelling and redness and history of gout. Usually knee aspiration is the best way to find out to be honest with you..

    14:18 Paul: “So even if I don’t have it, I still have to tell my wife?” Dr. Greggs: “That’s right, Paul. You should have thought about that before you started cheating on your wife with sex workers. If you don’t tell her, then you’re going to jail. And if you don’t have HIV now, you’re probably going to get it in prison. This happens a lot when men come here after doing what you’ve been doing. I’m very sorry, Paul. It has to be like this. We have all your information already, and I’m going to have to pass it along to the constable. So sorry. So would you like to proceed.” Paul: “Yeah. I think I need more time to think about it. And yeah, Paul isn’t my real name, so I don’t really care if you tell the police or not. I was just coming to the clinic to see about getting tested, but now I’ve changed my mind.”

    9:25 Why does the practitioner feel like they need to ask all these personal questions? Just give the patient the damn test. If a patient goes SCUBA diving, do you ask them if they checked all the safety equipment first? What about if they go skiing or riding a bike? _“OK, Paul. Thanks for visiting the clinic today. About how many times per week do you ride your bike? OK and each time, do you wear protection? Helmet? Shin guards? Paul, who would you call if you fell off your bike. Your mummy? Or your wifey? OK, Paul, when you go outside on a rainy day, do you wear protection from the rain? Raincoat? Umbrella? Boots?”_

      This is actually funny! No hate buddy! You can always skip the questions. This is an example only.

  • @DaRyteJuan
    @DaRyteJuan 11 месяцев назад

    4:28 *Hope the Patient will cope with a positive result* I like how she advises the practitioner should ask if the patient has “good financial support.” And if not, then what? A patient who thinks they might be positive but can’t afford to pay for any treatment will probably just bail out of getting tested. They might even get themselves so worked up worried if they were positive they might attempt to commit suicide. They might actually commit suicide and turn out to be HIV negative. Yep. Asking the patient how they’re going to cope financially is really going to help.

    Asking sexual history questions is EXTREMELY OFFENSIVE. I recently went to a male PULMONOLOGIST who had the NERVE to ask me if I was engaging in MSM. I FIRED HIM IMMEDIATELY. I also am escalating his OFFENSIVE and DISCRIMINATORY conduct to the PRESIDENT of this medical facility. And you’re going to ask every FEMALE patient if they have sex for money or drugs. GO TO HELL!

      Hi Jie Fei Yes, I am applying to the LIFT program in Sheffield even though my aim is to join core surgical training eventually. It suits my needs as I prefer a bigger city so that my fiancé can find a job easier than small rural areas. I did go through the rotations in the excel sheet and it fits my choice. Personally, I must do a rotation of medicine, surgery and obs and gyne so that I am well rounded in the event life ever sends me to a rural place. Imagine not knowing how to deliver a baby in a kampung. Plus, both the FY1 and FY2 are based in Sheffield. I dont want to move or even drive more than 45 minutes to another hospital in my FY2. Hope this helps
