The Answer should be D Key Features: Age: 67-year-old man. Presentation: Sudden onset of pain, redness, and swelling in the right hand, with the pain being severe (8/10) and increasing with use. Physical Exam: Erythema and tenderness in a specific joint (between the 2nd metacarpal and proximal phalanx). Lab Findings: Serum uric acid levels are normal. Synovial fluid analysis shows monosodium urate crystals. Diagnosis: The presence of monosodium urate crystals in the synovial fluid is diagnostic of gout. Even though the serum uric acid is within normal limits during the attack, it does not rule out gout. Acute gout attacks can occur with normal uric acid levels. Management Approach: First-line treatment for acute gout attacks: NSAIDs (e.g., indomethacin) are typically the first choice if there are no contraindications (e.g., history of peptic ulcer disease in this patient may make NSAIDs less ideal). Colchicine is another option and is particularly effective if used within 24-48 hours of symptom onset. Corticosteroids (e.g., prednisone) may be used if NSAIDs or colchicine are contraindicated, which may be more suitable here due to the patient's ulcer history. NSAIDs (e.g., indomethacin, ibuprofen) are commonly used to treat acute gout attacks, and they work by inhibiting cyclooxygenase (COX-1 and COX-2) enzymes in the arachidonic acid pathway. Given the patient’s history of ulcer disease, a COX-2 selective inhibitor (e.g., celecoxib) might be preferred to reduce the risk of gastrointestinal complications while still providing anti-inflammatory effects.
