Ahh thanks, I now finally understand what causes my ankle to clunk like mad. The anterior lateral ankle ligament area has always been tender and I've rolled the outside of my ankle tones. Walked exclusively on my toes for years. I only realised recently that it wasn't the case that my right ankle had a much worse equinus, it is slightly worse as all my weight is usually on my left leg. What's actually been going on is, my left glute max has just become dormant so don't extend my left pelvis, and this allows me to have some heel contact on my left leg. However it's been destroying me. My right glute is already in a position where It pushes my pelvis around to the left and any extension whilst on my right leg adds a tone of torque to my right knee. I've essentially been stuck on the outside of my left leg, with 90% of the weight bearing on that leg. Physios actually told me to work on external rotation as it kind of looks like my right leg is internally rotated and more medialy positioned. This naturally made things far worse. Also being given exercises based around a flat foot, when I can't achieve that position without compensating somewhere else was just pointless. They had me working on the already strong side, primarily with additional single leg exercises that were useless as I was essentially doing them wrong for my body. I believe I'm on the right track now muscularly speaking, and am doing single leg pelvis extensions on my left side, whilst I feel the glute for activation. I'm also not stopping my heel from rising and am using that glute to shift my weight forwards and back over the foot. Could I ask you, what is the better choice for me: An arch and heel support orthotic with a slight overall raise, or an actual heel elevation wedge? For context my right heel sits about 3.5cm above the floor and my left about 2.5, but my method of testing was, going as far up onto my toes as possible, then keeping the knee straight and just slowly lowering till I was just hanging on the bottom range. So my left glute max weakness likely meant my center of mass was shifted less far forwards over my left foot, influencing the result. Also during regular walking I feel zero activation at any point in the calves and my toe push off is achieved entirely by the glutes. Should I feel some calf activation? And if so at what point? I'm purely guessing, but I assume normally the glute pushes you forwards to perhaps the midfoot area and then the calf powers the rest, rather than the glute the area between the ball of the foot and the toes and then my calf at best pushing the big toe and maybe a little bit of the 2nd and third toes into the floor, pushing me off diagonally. Also glute bridges seem to just not work properly for me, simply straining the hamstrings, would doing glute bridges without flat foot contact, only the balls of both feet be a good idea or not? Sorry for the long post, I just thought I'm better off asking on here than at my doctors/physios, they just ignore what I have to say and give me generalised exercise sheets not catered to my needs at all. I even said stated I basically have no clue how to do the exercises properly and asked for a demonstration. During which I said I can't feel any glute activation whatsoever despite appearing to do a bridge semi normally. Anyway enough of that, needless to say my frustration with the medical care I've received is immeasurable. I'm 26 and it's taken 23 years of saying the same things to get anywhere. I am hoping to have tendon surgery soon finally.

Hi, just got my first orthotics at 67 after feeling aching mid foot. I also have a “ bump” on each foot but no pain but I am now a slow walker as my 3rd and 4th metatarsals are constantly sore. (They do feel better when I stretch my calves). If I use the tennis ball - how often - daily? Also, if I manage to get more movement do I stop when that happens or is it now a lifelong practice?
Hi Ann, I recommend using the tennis ball routine for 3 weeks then review. Most people get into the habit of doing it daily (like brushing your teeth) and carry on. If there's no ill-effect, you will likely benefit from continuing indefinitely. Best results come from using the tennis ball for 1-2 mins first thing in the morning then again last thing at night. Having the ball by your bedside makes this convenient to do. Good luck! Ted

@@TedJedTBD Hi Ted, using the ball BEFORE I get up is a revelation! It gets my feet warmed up and ready for the day. I was stretching etc at the end of the day when my legs and feet were already tired but the morning routine is much better as my feet are less sore by the end of the day.

I have just been diagnosed with Calcaneouscuboid arthritis after suffering from Peroneal tendonitis. I was also told that a fusion should be done but I’m not going down that road until I’ve tried everything else. I am seeing a Podiatrist and am about to find out if he does this treatment. Hopefully yes but I have had it for a couple of years now so fingers crossed

My name is Lucas Wanderley, 28 years old, aiming to train and perform speed ice skating and currently living in Sydney, Australia. I have been struggling with some weakness, unbalance and pain under certain charge in my left leg. And it is noticeable that the fibula head is slightly sidewards. I went to see an orthopedist today and he immediately recommended surgery. Not even exercise recommendation, change of diet, nothing, straight to surgery. Insane.
When you consult a surgeon, their expertise is surgery ... 'if your only tool is a hammer, then everything looks like a nail ...'

Yes, this can certainly release the plantar tissues that are maintaining the high arched architecture. Flattening out completely though? I'm not so sure. Definitely helps in improving mobility for those rigid restricted cavus feet.

You need to make that assessment BEFORE you start mobilising any joint. Plates and pins usually (but not always) means that mobilisations/manipulations are contraindicated. This will depend on what your treatment objectives are too.

AS an Optometrist fellow doctor i understand what you are talking about, but i also can think as a normal person. I have worn Vibram Five finger shoes for helping me treat plantr faciitis and after 6 months the problems in both of my feet has resolved. i ended up also developming full mobility in my big toes in both feet and developed much stronger toe muscles.
Great news to hear! I SEE what you're saying about the mobility and strength improvements too, Richard. There is plenty of evidence supporting your experiences BUT the challenge is the differences in adapting to Vibrams varies soooo w i d e l y and is impossible to predict. This is where people run into trouble - they don't adapt at a rate that suits them specifically.
DEar Ted; Thanks for responding to me. I would like to know about the difference between the etology of dorsal bunions vs. traditional lateral bunions that cause pain on the side of by big toes. i have noticed that my left foot that is bigger was more affected by both plantar faciitis and ended up with a much bigger doral bunion. Now all my three sisters ended up with significant lateral bunions, however i as the only male ended up with stage three doral bunion in my right foot and my left foot that was slightly smaller never developed as serious a case of plantar faciitis as my left foot. My sisters have not deveoped plantar faciitis. Our mother has bunions as well. So our common heredity may have a common effect on our family problem. However, for me i put the blame strictly on my wearing traditional shoes. i am finding i am now feeling some joint sensitivity in my left knee. Not bad, but livable as i continue my recovery. I can live with it. I can also continue to wear only five finger shes for a few more months . I will try to keep you informed of my PERSONAL recovery results.
