This video follows the reference denture intraoral scans in order to fabricate 3D biteplates to obatin a bite registration. The 3D Biteplate uses Ivoclar's Gnathometer CAD in order to take a final impression and do a gothic arch tracing of the patient's jaw movement.

This video shows the reference denture technique. With this technique, the provider needs to take a wash impression on the maxillary and mandibular dentures, then scan the dentures outside the mouth. The dentures are put back in the mouth and the bite scanned . Due to the amount of wear on the teeth, I will have to design a 3D biteplate in order to obtain a more accuaret bite registration.

I had to integrate some analogue procedures in order to be able to sit the locator housings in the model instead of picking them up in the mouth and have them lock in. I took an intraoral scan of the Maxilla but did not like the borders of it, so instead I took an impression of his old denture and the wash impression looked a lot better and was very retentive. The lower scan came out very well.

Hi Fernando - Great video, going through a watching a few of yours. Would be great if you narrated them, and explained a little why you do certain things. I hope you don't mind me asking a few questions. :) When you use the show local cross-section tool to adjust the extensions, what particularly are you looking for? With PVS impression material for the final impression, are you using a 1mm gap as you have set in the video? When you smooth the extensions towards the end, your smoothing tool is set to the strongest setting for depth and size, how are you judging that you are not trimming back too far, especially are you were so precise in drawing the border-line and using the cross-section tool? Thanks very much - if you get the time, would you please try make a full video on an Ivotion mono-block setup from start to finish. I'm fumbling my way through at the moment, and no-body seems to have done a nice comprehensive video on it. Alternatively, if you could recommend someone with some good videos on it, that would be great. Thanks again, Steve - Denturist from New Zealand
@@zimscubasteve hi Steve, in regards to the cross section, I am looking to draw my line 2mm below the sulcus to allow room for the material during the final impression. Even though I draw the line, the software can add more material or it seems that I don’t have enough space and that’s why I use the soften tool to take away. We Ivobase all our cast partials. That makes them a hybrid denture.

i have a question, in regular alginate impression we record in functional state as well by retracting the cheeks n rotating them even perform tongue movements does the scanner record the mucosa in functional state as well?
The scanner will only scan mucostatic tissue since no pressure is being applied. However, you can add pressure into your denture design for retention. For example, a common addition is the palatal seal.

I barely add clasps when I design partial digital dentures as long as there are undercuts on the lingual or posterior aspect of the teeth.
how accurate is the manual alignment? is it enough to eyeball it? how is the accuracy of intraoral scanning vs. scanning an impression? thank you for sharing the video!
The manual alignment in the Trios software can be a bit off from time to time, but the manual alignment in the design software is very accurate. The accuracy would be fairly close. Scanning with an IOS is just a hair off from scanning with a desktop scanner. I am glad you are enjoying the videos.

Hi 👋 Can you please tell me how can I improve my denture design at home? Because the lab I am working now doesn’t allow us to use the facility but I really want to improve my design fast.
The only way to improve digital denture or any other type of digital design is by doing it. If the lab you work on have the software in a laptop and they can allow you to take it home, then that’s a good way to. If not, the only way is for you to buy yourself a fast gaming laptop and the software, but is not cheap.

But I will try to buy one as soon as possible. This is just a normal CLD order. Gingiva is Ivotion base for milled teeth, (I use the Ivoclar Mill, so for me I use the cam5 manufacturing process. For the Anatomy I choose artificial tooth, Ivotion multi and cam5. Then I bridge all the teeth. 👍🏻👍🏻

This is so disheartening to watch. I was this person. Orthotics are a crutch, not a solution. You’ll spend years and countless dollars in vain. Flat feet are normal. It’s the shoes not the inserts that are the problem. Invest in barefoot shoes, that allow your toes to splay out (which supports your natural arch). Zero-drop (no raised heel) and minimal cushion (as high cushioning makes your feet weaker over time)… spend $10 on some toe spreaders. Do physical exercises to strengthen your feet/toes. Good luck.

Question around the 42min you start playing with the occlusion contact map and areas, what colour is best option? and how to set it up Reason for asking is, been desinging and have so many problems with my clinicians about not having proper contact or my printed try in wobble at pre molar areas and 7 are in air but on screen it shows in perfect occlusion??
I adjust the teeth until I see a bit of red and more orange. Then I go to the sculpt anatomy stage and adjust the bite by grinding the bite at least three times pre denture. Or maybe the Dentist has taken a wrong bite registration. Hope that helps.
@@smilehighclub6176 Okay that helps a lot, I had mix responses to how they use the map and what colours mean. Do you find that when you morph to much on a tooth it feels like the antagonist tooth shifts down into that space ? or would that not be possible? I ask because when I play around and test it seems when morphing a certain area to yellow then morph even more it tends to make massive red or blue area🤔
Where do you get the gnathometer if it doesn't create a file ?
The Gnathometer CAD can be bought from Ivoclar Vivadent. The software only makes the baseplates for them.

Hello Dr Sohaib, there are many different ways on recording a bite registration digitally. You may use the denture the patient is wearing, you can use a centric tray or you can use bite rims. Hello Sir, I’m a technician currently started to use 3shape, may I ask the material setting of the plate, whereby base thickness of 2.5mm, impression gap of 1mm and relief of 0 is standard? Or differs from case to case? Thank you
I usually leave the base thickness to 2.5mm in order for the tray not to break in the mouth. In regards to impression gap, I leave it at 1mm to allow space for the impression material. I don’t use relief so I leave it at 0.

Sorry one more question Sir, are we able to raise the bite, if the VD obtained from centric tray registration is way too low, for example around 30mm only. As if the VD is too low we can’t set the Gnathometer in.

It’s a new feature on the latest software. You should always keep up with your software so you can enjoy the ease of use each upgrade brings.

You need to use a Centric Tray to be able to take a preliminary bite registration. Then you need to fabricate a 3D Biteplate in order to take final impressions and a proper bite registration at the correct VD. You rescan the 3D Biteplate and design your denture try in. Hope that was helpful.
Hi there, After you took preliminary (primary) impressions with the intra-oral scanner, do you use the model to fabricate bite blocks/special trays to take secondary impressions? or do you proceed with preliminary impressions without secondary impressions? Love you passions in dentistry!
Hi Tay, it really depends, I think I’m this case I fabricated a 3D Biteplate(gothic arc tracing) in order to take final impressions and do a bite registration. Hi Andy, yes I print try ins. I have a Nexdent 5100 printer and use their try in material in shade T1I, which is close to an A2 shade.

I don’t border mould. I just take a wash impression, but I let the patient mould the material to their muscle in the mouth by using the Dr. Abe technique on final impressions.

You can use existing dentures, or take a preliminary bite and then use bite rims, or pin tracer. With digital dentures, you can have a new set made in 3 appts(including a try in).

Hello Miss Spaz, yes it can be done. Not sure what you mean by have them sent to you. You will have to go back to the dental provider to have them inserted.
Why is there such big discrepancy in gingival reveal of lower incisors?
It’s an immediate denture with little space for the anterior teeth.
Digital dentures can not due what conventional dentures can do.. such as cast partial and process to implants !!
Digital dentistry has made fabricating anything simple and extremely accurate including working with cast partials and implants. Have you tried it?

We studied in school that we beed a functional impression , we also need to move soft tissues to avoid future dislodgment and better fit. How can we get this results with the scanner?
Wait what? Move soft tissues? Nah man. I lost all my teeth at age 22 due to a severe illness that I wasn't expecting to survive from. And they never had to move any soft tissue around...
what is the point of this if you already had the vertical dimention stablished and also already articuladted? its just pointless.
The point of this is to make sure I am able to establish a proper VD. This patient came in with extremely worn down denture teeth and had extremely over closed VD with the Md in protrusion. Therefore it needed to be corrected, and this was the best way for me to do so. My patient and I did not find this solution to be pointless as it allowed my patient to go back to a more natural VD, improve the appearance as well as the chewing abilities.

@@mistycmcana953 the 3D biteplate is a Gothic arch tracer. A well trained Denturist is able to provide any patient with a proper VD by using either wax rims or pin tracers or in case of digital dentures they can use existing dentures. In this case I chose this as the best option for my patient. Unfortunately, it is impossible for anyone to say if something works or not without having seen the patient before and after the dentures were made.
@@smilehighclub6176 brother , in our office we use jaw tracking motion , along side with a mounted sideby side conebeam with a modified gothic arch plate to track real motion of what you might be suggesting as your 3dbiteplate , and i asure you you are not in centric relation . do abit of study and you dont even have a propper impresion to have a well measured lower oclussal plane. im not going to discuss this with you but to me its pointless. awell trained denturist does not compare to a well trained gnatologist period.

Fantastic! question I am about start working on the digital denture. do I need a scanner E4 ? or if I have only trios 3 basic wire can work for me both as an intraoral scanner and as a scanner for other models ?
The Trios 3 basic can be used as an IOS and to scan models and impressions. You can place your Trios 3 on a cart for portability(I use an Ergotron cart). The E4 is convenient though, you place your models or impressions or dentures to copy on the plate and the scanner does the rest(hands free). The faster the graphics card the better for the E4. It is an extra expense though. You would have to weigh the pros and cons and see what will work better for you. Let me know if you have any other questions. @@smilehighclub6176 I really appreciate your advice. although I work in a dental lab for many years work with 3shape for almost 6 years designing crowns, bridges and implants, I have no enough experience with digital dentures in the clinical and lab sides. I recently graduated from the denturist program. I took my board exam last month and waiting for my results. Now is the time for me to think about my future, I want to establish my work on a digital base, but it's hard for me to get all the necessary tools at the beginning. for start I want to do mobile dentures. I can't get the scanner and IOS both together. so here is the question, can I use the trios3 basic the wire one which is cheap, for IOS and take it out as portable and also use for scanning stone model? I really appreciate your help.

Yeah, this was a Timelapse of the setup. Of course this is now old technology, as all my dentures are now done digitally. 👍🏻😃
