It was a oil rig then it was modified for gas my brother worked for occidental petroleum and fortunately he was on leave when the disaster happened

This sends chills down my spine. I’m not familiar with Top Drive system, in my day roughnecking in the North Sea (late 70s) we didn’t have it. And looking at this, I’m glad we didn’t. Rule No.1 was be aware of where you’re standing at all times, relative to what is going on around you. Poor guy, cut down in his prime. RIP brother.
This is a very old top drive unit, one of the first Canrig made. It had far less safety measures. What they did is use it improperly, put torque on the quill connection, put weight and then released the pipe handler lock, and pipe handler was sharing the torque of the quill. Newer versions will not let you do that, open the lock while the quill is under torque. There was an program safety update for this unit too, but was never installed. Sad.

Bad thing is his mind was alive for at least 1/2 seconds. You can see his lower arm wrap around the bottom white part. His internal organs were completely crushed in a flash. R.i.p man. God rest your soul.
You see that after the smash his arm was still holding on to that piece and then slowly falls away. He wasn't killed instantly. But he honestly probably suffered no pain as he saw his entire life flash before his eyes in that last 2 seconds as he took his last breath into the sweet relief of nothingness. I didn't know you brotha but I'm glad you got to the other side quick. Some us just slowly dying and it is a fucking ache. See you out there buddy

Locking pin on the top drive. Supposed to lock while in connection mode so when they break out of the stump the top drive doesnt spin around like crazy. This was a malfunction. We heard all about it years ago

I think his buddy behind him was trying to warn him about being in the pinch point he taps him on the shoulder but he ignores it and keeps talking to whoever he’s talking to , then proceeds to get smashed in between that metal infrastructure he was most likely killed instantly. As sad as this is it’s a lesson to always be aware of your surroundings no matter who’s talking to you a conversation can be put on hold your life on the other hand cannot.
Situational awareness is something extremely few people have.

He looked badly injured and in shock , he probably needed medical attention & to get someone. You can’t go check on a dead guy , there’s no way anyone surviving that ,
if anything is moving, can move, or may have a 1 in a million chance of moving ... figure out how it might move before going anywhere near it. then stay away anyway.
You are absolutely right, use common sense to protect yourself. But the things that work well in day-to-day life are not much applicable to highly hazardous industries. In Oilfield industry it came at huge cost to depend on common sense, therefore there were put hundreds of procedures to prevent life or asset loss by means of introducing multiple controls on the path of the hazard.
