It's sad, Sarah didn't want to ever get pregnant, but John's fiance did. So his fiance got him to go through with their wedding, because he wanted to be a father. He could never leave her for Sarah in the future, not without losing his child. His ultra rich and powerful father in law would have made mincemeat out of John if he had deserted his wife for Sarah. And Sarah didn't seem too broken up when he decided to marry someone else. She had her own detective agency after all, and that seemed to mean more to her than he did.

I recently rewatched this movie (as an adult) with a much different perspective of life and of affection and personal relations. Now I get this movie. Back in 2014 when I was a teenager I didn't.

I had a destructive all consuming chemically explosive love like this for 7 years before he left me broken and devoid of a willingness to go on. 20 years later i never regretted it indeed how fortunate i was to feel this at all. I am happily married to a kind man who loves me and i am content.
