I am currently in Menopause and my sixties endometriosis is running rampant in my body. The pain is unbearable and gobs of endometrial uterine tissue(?) are in my system and sometimes being excreted.

    So insightful! Should patients request ARID1A and mitotic rate testing on tissue samples from surgeries? Would be amazing to get clarity for those getting surgeries from other excision specialists who don't have this oncology perspective.

    • @DrSteveVasilevMD
      @DrSteveVasilevMD 6 месяцев назад

      These are all part of cutting-edge notions with a strong basic science base but no clear clinical proof of outcomes benefit. Having said that, the information can contribute to helping make more informed choices after surgery. The mitotic index is something that can be requested from pathology and may provide insight into whether or not recurrence is more likely. It is just a matter of counting mitotic figures under a microscope by a specific protocol. The mutated ARID1A marker is, as mentioned, something that overlaps cancer and non-malignant but potentially more aggressive endo lesions. The problem is that this test is reserved by the current specialized testing companies (based on FDA approval) for oncology cases for the moment. As more data develops, this may open up. As a proxy for ARID1a molecular testing, a stain (called immunohistochemistry) can be used to measure if the tumor suppressor gene ARID1A is expressing itself normally or not (i.e. proteins from this gene's signals in normal or mutated state can be tested for). This is not standard of practice but can be ordered at an additional expense. Unfortunately, this is not likely going to be covered by insurance in most cases, but it is not wildly expensive. The bigger and more important question is what these tests can mean in terms of treatment options. There are no clinical studies yet to answer this question, and it may be a long time coming. However, positive results may mean closer monitoring is prudent, especially if there were elevated markers prior to surgery, like CA125 and CA19-9. Also, meticulous attention to lifestyle and diet can reduce excess estrogen levels, which may impact recurrence rates (also not proven but poses no risk and generates overall health benefits). This is easier said than done, but might be more top of mind when facing a likely higher risk of recurrence. In addition, positive testing may stimulate more consideration for post-surgical hormone suppression options, using the least toxic or lowest side-effect options available. Overall, this kind of approach will increase as we progress further into the molecular personalized medicine age for early diagnosis, treatment options beyond hormones, and postoperative monitoring.

    www.ncbi.nlm.nih.gov/pmc/articles/PMC7979491/ Widespread myofascial dysfunction and sensitization in women with endometriosis-associated chronic pelvic pain: A cross-sectional study V. T. Phan, BS,1 P. Stratton, MD,2 H. K. Tandon, BA,1 N. Sinaii, PhD,3 J. V. Aredo, BS,4 B. I. Karp, MD,2 M. A. Merideth, MD, MPH,5 and J. P. Shah, MD1 Importantly, the neuro-musculoskeletal examination revealed that all women also had widespread myofascial dysfunction with palpable myofascial trigger points and low pressure-pain thresholds in nearly all muscles tested. Similarly, widespread and regional sensitization were observed in most women in addition to low pressure-pain thresholds over the interspinous ligaments On the neuro-musculoskeletal exam, all women had widespread myofascial dysfunction as evidenced by myofascial trigger points in more than two-thirds of assessed regions. Fourteen (47%) of 30 women had myofascial trigger points in all 26 assessed regions. A low pressure-pain threshold was observed in all of the assessed myofascial trigger points (Figure 3). Most women had widespread (17/30, 57%), thoracic (21/30, 70%), and lumbosacral/pelvic (18/30, 60%) spinal segment sensitization. Cervical spinal segment sensitization was manifested as low pressure-pain thresholds; in addition, orofacial pain was reported in 21/30 (70%) or recurrent severe headaches in 23/30 (77%). The pressure-pain thresholds over the interspinous ligaments were generally low within the cervical, thoracic, and lumbosacral/pelvic regions (Figure 4). Overall, sixteen (53%) of 30 women had low pressure-pain thresholds over all interspinous ligaments. DONT LISTEN TO THESE TWO. They think your loved ones endometriosis is going to get better by riding a surf board ? Most if not ALL women with post endometriosis lesion removal have widespread myofascial trigger point dysfunction.

    I had knee surgery 4 days ago. as soon as I went into the OR the DR said I would moved to the table, but, 1st, he would give me a little bit of medicine. I looked down at the tube and saw white liquid I, and I went out. I never remember getting on the table, having my arms put on the boards, never remember being asked to breathe w/the mask on. I think he put me out too quickly. Did I do all that other stuff and just don't remember? I just remember waking up in the PACU when the surgery was over. I was having a nice time looking over the OR and seeing all the machines. In 12 days I will have an endoscopy, so I'll see the difference. This will take quite a while as the docs will try to close a fistula, endoscopically

    Short but good. Have had everything from none, ether, sodium Pen. Have had 20 surgeries requiring anesthesia of some sort. My last surgery was for total right knee replacement. I had a Spinal without sedation. I was awake for the whole process. It was by far the most enjoyable anesthesia and surgical procedure I have had. It was a new life's experience. If I need surgery in the future and a spinal is used I will do it again without sedation. The main reason tor anesthesia that renders you unconscious is to prevent you from developing PTSD. I have seen and done so much during my 83 Orbits around the sun. I have no PTSD nor am I susceptible to PTSD. If I was, I'd already have it. I was in the military during Vietnam and I was a Cop. Do what you think is best for yourself. General Anesthesia at level 4 (Unconscious and on a ventilator) is more dangerous the older you are. Avoid it if possible. Versed/midazolam is used to keep you from forming new memories and also causes the loss of some older ones. Avoid this drug, it's not necessary.

    Hi :) Thanks for uploading this video! I'm 3 weeks PO and would like to recommend this video to anyone watching... the video is accurate. My experience with Endometrioma excision/cystectomy/bisapingectomy and unilateral oophorectomy with the anesthesia, all went smooth. Had Propofol + Sevorlorane gas and Peripheral Nerve Block. PLEASE MENTION THE PNB when you speak to your anesthesiologist! It can make all the difference. They gave me a RX for opioid based pain medication I NEVER needed to fill it! Because of the PNB. I only took acetominophen (which doesn't work GREAT but I still took as recommended), they cleared me for ibuprofen, and I got clearance to use CBD. The PNB can make a huge difference in your pain management/amplification result coming out of Post Op! Be sure to ask for it. :) !!! Also - some recommended that having a cold beverage ready with lemon and honey may soothe throat irritation. I personally had ZERO irritation from the tracheal tube! I dealt with my surgery 100% alone , except for a wonderful team. If you are going through it alone, STAY STRONG, you can do it! Anesthesiology has come a long way. - one more thing If you ever HAD general anesthesia, and did not have nausea, I would probably say to research the anti-nausea patch. It is an option they can give you but you want to decline it if you can, because that patch has the side effect of making it slightly more difficult to urinate. This can complicate when you have your CATHETER removed. I did not have the Anti Nausea patch and ALSO - since it was a day surgery, I had a slimline (NON FOLEY) catheter. I had an easy time urinating on my own when I got home. @Icarebetter thanks for the upload and if you have not, perhaps make a video about CATHETER, I had only minor irritation but both ANESTHESIA AND CATH frightened me, but there was no reason for it! Thank you very much. 4:30 I experienced a minor complication from the PNB which is temporary numbness (neuralgia) of a specific muscle in the thigh (Meralgia Paresthetica) but it usually resolves in patients who have PNB/robotic laproscopy within 3-4 weeks and mine is resolving. It is not an uncommon complication and not terribly difficult to deal with, as is reversible.
