Follow up for Full Arch Wax Up: Adapting Cervical and Embrasure areas of Diagnostic Wax up
Просмотров 5274 месяца назад
This video is in response to a very good question by Dr. George who wanted to know how to better adapt the cervical and embrasure areas better. Having done this kind of thing for 30 years, I realize I can get sort of cavalier about the details sometimes knowing that I can fix it in the provisional restorations, so I appreciate the question and comment. So here I am going to get into the weeds a...
ExoCAD: Quick Wax Up for 3 Unit Bridge
Просмотров 5144 месяца назад
This demonstrates how quick and easy it is to make a model to use for provisional restoration of a 3-unit bridge. You can do this yourself or we can train an assistant to do it for you, but you will have the best provisional model that you could ever hope for in minutes. Combine ExoCAD with a 3D printer and you take control of your digital workflows.
Full Arch Wax Up

This video demonstrates a full arch diagnostic wax up using ExoCAD to determine if this treatment is feasible to establish the requirements for a stable occlusion. With this information I can do an intraoral mockup to show the patient how their new teeth will look in their mouth and formulate a treatment schedule that will show the timeline and payment schedule for completion of the case. When ...

This video illustrates how to use ExoCAD on a daily basis to help in treatment planning. Problem list includes: 1. I am seeking an orthodontic opinion and help from my orthodontist friend. 2. Large CR to CO discrepancy 3. No anterior guidance 4. Rubbing on posterior teeth 5. Lower right in provisional restorations for caries control. 6. Breaking upper and lower posterior teeth due to the rubbin...
Adding Surgical Guide Irrigation Tubes in Meshmixer

It is very important to get as much irrigation to the osteotomy site as possible. Properly positioned irrigation channels will allow the assistant to push cold sterile saline from a syringe straight to the site.
Locator Denture on 4 implants
Просмотров 216Год назад
Locator abutments need to be parallel to function well and if using 4 locators they need to have a sufficient anterior to posterior spread so they resist rocking as much as possible. Parallelism can be difficult to achieve within the confines of the patient's anatomy but when possible it will create an implant over denture that has incredible stability and can be used as a cost effective altern...
Occlusal Equilibration and Wax Up 7-10 in ExoCAD
Просмотров 2,5 тыс.Год назад
We need to do trial equilibrations prior to adjusting patient's occlusion intraorally. This can be done digitally in ExoCAD with a similar result and decision making process that we have used for analog processes in the past. Once equilibration is achieved a wax up can be made for cosmetic changes. This video show how we apply the Dawson concepts of occlusion to work up a case in ExoCAD. Models...
Prosthetic Screw Location Guide

Crown Control via ExoCAD

A huge practice builder is consistency of your product. NO REMAKES! Learning to design your own restorations will give you control over this parameter. Non-adjustment delivery is the primary objective for every procedure and control of quality output gives that end result almost every time. This entire process that took 42 minutes in real time has been time lapsed to about 5 minutes. You can pa...

Guided Dental Implant Surgery: 2 Implant Locator on Bone Guide with Fixation Pins
Nicely done Dwight.

Can't youjust do shell temps in exocad, then print then reline?
Sure, you can easily make and print shells. Shells are often difficult to fit over your preps and require significant hollow grinding out the intaglio. During reline it can be difficult to maintain vertical dimension as well as horizontal cant or plane of the smile line/plane of occlusion. Printed shells have the added disadvantage of not being as strong going through the hollow grinding process. If I were to do shells and I have the luxury of a PMMA mill, I would prefer to have the shells milled in PMMA. I, like many others, prefer to work from sequential PVS models and make a few temps at time. I have tried shells so many times and without exception I prefer the sequential PVS matrix method.

Thank you for your reply! I have been a qualified Prosthodontist since 1994. I know exactly what to do the analogue way but digitaly I am in my infancy. So my question is when you superimpose the library tooth on the natural one, how you get rid of the wax portion that overlaps the cervical area of the natural tooth. In other words how you make an onlay type of wax up and how you make a printed model of the wax up. I think that a video of the wax up process from start to finish would be helpful for other people too! Also in exocad do you prescribe wax up or mock up?
In essence I can do one of 2 things. As long as the changes are reasonable and practical, I do not need to make it 100% perfect, but I can go in and adjust the cervical and embrasure areas to match the pre-op cervical and embrasure areas. I will make a video to explain this later today or very soon if not today.
Perfect! Thank you once again @@dwightpate7629

I see from your name you must be a dentist.
You manipulate each tooth to make the cervical areas match up. This is important because you cannot transpose the cervical areas. You also must make certain the changes you make are practical restoratively. You cannot overly reduce a tooth nor grossly distort a tooth. Additionally, I am always thinking about neutral zone, vertical dimension, envelop of function, anterior guidance, curve of Spee/Wilson, etc. There are usually compromises but they must be reasonable, practical and functionally realistic. When you put this all together properly, patients will tolerate the changes quite naturally. Are you a dentist or technician?
Yes that's what I am asking, but I mean how you do it technically in exocad, as I have been training in the software for some months now. Thank you!
Not sure I understand your question but I make sure the cervical emergence profile of the wax up teeth are aligned with the patients teeth. Is that what you are asking?
Thank you very much for your very infomative video Dr Pate and also for the way you are dealing with occlusion! Can you please explain how you incorporated the wax up to the existing teeth? Thank you very much and keep posting your unique and excellent work!
Man i was trying to not comment but...after 3rd minute all went downhil..you aint Dental technician nor a dentist..if you wanna to not waste time in articulator mode just skip it and go with two stone models in static occlusion..if you want to use vartyculator then learn atleast basics Biomechanics of tmj and basic anatomi so you can imagine what artyculator represents
Hey, sorry that you do not like my approach. I use the digital articulator the same way as I have used the analog articulator successfully for 30+ years. I realize the analog and digital articulators are a fairly basic representation of the actual 3D anatomy but it has worked well for many dentists, like myself, for many years. I suppose vartyculator is a typo maybe but for sure I do not know what that is? I think I have a pretty good understanding of TMJ biomechanics and anatomy although I admit, some may have much more knowledge than me and possibly you are in that group.
Great video, I am currently looking to bring my equilibrations to a digital platform. Did you take lessons to be able to work the exocad steps? If so then where? Did you use a digital facebow to bring the scanned teeth to a digital articulator? Did you scan an open bite in CR then close digitally? Do you have a lab finalize your wax ups before provisionalization? This digital work flow is very exciting. Keep up the great work.
Hey, I am just now seeing your questions, sorry for not being on top of follow up. I will start to check this area closer. Lessons? I am pretty much self taught as lessons were not available when I was starting out. I teach courses at my office and can do some one-on-one sessions by special arrangement. Facebow? I use a 95 to 100 mm arbitrary hinge axis in some cases but in the case of occlusal trial equilibration the best is to measure the hinge axis with a square. Still slightly arbitrary but so is the facebow. I do scan using an anterior bite stop in centric relation as close to first contact as possible then verify that the 1st point of contact matches the 1st point of contact recorded in the mouth. Lab finalization? Nope, I finalize my own. Much lower cost and 100% control and get what I want. I can 3D print models to allow segmental provisional restorations in a very controlled workflow. I offer courses at my office starting at my office, email me to arrange dates or times: drp@drpatedme.com
Plase share the spreadsheet 🙏
Hey, I am just now seeing your questions, sorry for not being on top of follow up. I will start to check this area closer. Great and helpful video thank you
Fantastic video Dr. Pate! Thank you for sharing your video. Can I ask how you transfer your digital trial equilibration to the mouth?
Hey, I am just now seeing your questions, sorry for not being on top of follow up. I will start to check this area closer. The trial equilibration is only to predict the feasibility of being able to accomplish the equilibration intraorally. The trial equilibration allows you to predict the beginning but the final equilibration has to be done in the mouth with the knowledge that you can get to an occlusion that satisfies the requirements for a stable occlusion, so the trial equilibration predicts the beginning and the end. Currently I cannot print say a reduction guide for the equilibration. Also realize that all semi adjustable articulators currently available have a straight-line condylar path and this works very well without the more complicated gnathological approaches or even jaw movement data collection. Because the patient disarticulates the posterior teeth much faster than the 25 to 30 degree condylar setting that I use, the intraoral equilibration is generally much easier in the mouth.
Great video. I’m using biohorizon pro taper series. How do I find out SP position on the final driver?
Hey, I am just now seeing your questions, sorry for not being on top of follow up. I will start to check this area closer. Blue Sky does not give you that information. There is a chart that we use comparing the drill length and the implant length to determine the stop position. I cannot paste that chart here but if you email me I will send it to you: drp@drpatedmd.com
Thanks Dwight, this is great idea!
