At 7.12 we see an AP chest without sufficient collimation as it seems to include the iliac crests in the primary beam. The patient should have the shoulders rolled forward to remove the scapula from being projected over the lung fields as we do with the AP. It also looks like 180cm FFD was not used. There is no reason why a supine chest should look like a lordotic chest as we see at 8.25. At 11.04 we see that lateral collimation has needlessly included much of the shoulder girdle in the phantom. Collimate to the skin edge laterally. Extending the arms like that will not roll the shoulders forward so the image will not be correct.

At 2.14 we see a lateral lumbar spine that began a lateral abdomen and masked to make it look like an acceptably collimated image. At 3.30 we see a FAILED EXAMINATION. This is because the ulna does not rotate as the wrist is rotated. So we look carefully at the broken ulna in the image and its appearance does not change between AP and lateral because it hasn't changed. This is an error a student radiographer will make and should not be taught of an instructional video for students. At 3.42 we see an uncollimated ankle xray with the wrong side marker. At 6.07 we see a lateral oblique of an elbow rather than a lateral - it is about 20 degrees off. At 7.11 we see no collimation for a foot xray - so the lower leg has been included in the exposed region that doubles the dose and the scatter. At 11.03 we see a lateral and an oblique of the thumb but no AP. The video represents the extremely poor state of radiography in the Indian subcontinent which results in poor assessment and treatment, and massive unnecessary radiation doses.

At 10.26 we see the hands of the operator being exposed to primary beam radiation. In the rest of the world, this would mean expulsion from the profession due to dangerous, careless behaviour. In the Indian subcontinent, it is standard practice. Not only is the hand exposed to primary beam radiation, the thigh on the other side will be emitting extra scatter. So there is massive dose from primary beam and massive dose from the scatter and all of that would not happen in the developed world.

Limit the radiation to the side of interest. Never include the unaffected side.

That is not good radiography though traditional. At 11.56 we see that all lumbar spine APs include the symphysis pubis and probably a primary dose to the gonads of both men and woman, which accurate radiography can avoid. Most AP lumbar spines we see in tutors from the Indian subcontinent are abdominal xrays that then have masks applied to make it look like collimation was used. At 16.51 we see that the spine is horizontal to the table which means a good lateral can't be achieved unless the postural positioning fails. At 20.23 we see the advised angulation for L5S1 as 5 degrees caudal. That angle will vary according to the positioning and the shape of the patient. Incorrect advice at 21.34 on beam divergence. Use it to enhance your lateral. All my laterals are perfect if the patient doesn't have a crooked spine. The laterals shown at 21.11 are not collimated. Most Indian radiographers expose lateral abdomen then mask the image to make it look like lateral lumbar spine.

At 4.02 we see that the collimator included much of the leg and femur due to little effort to collimate
