Thank you for the demo but I'm puzzled by what you said in 2:13, quote: "as we move buccal-lingually and apically we're actually creating a bit of an undercut as we go along, which we may or MAY NOT remove later when we do the radiused wall through the enamel." I was under the impression that not having any undercuts (cutting a cavity with a shape of a saucer) was one of the main principles of your technique, because this reduces the C-factor which allows injection molding to be performed safely. In fact, in the video, when you inspect the cavity and find those stress raiser in the buccal, the pulpal-lingual aspect of the cavity is undercut.

Thank you Dr. Clark for developing this technique. I've used it for many years and love it. But this video highlights a few things: the way this prep was cut seems different than how it was demonstrated in the past; it's much less "saucer" shaped and much more boxy. The saucer geometry was, if I'm not mistaken, the design element that reduced the C-factor; here I see more 90 angles (rounded and perhaps flared, yes, but at much more acute angles than the way the prep was described in the past). That's nice to see but I hope this doesn't impact the C-factor too much for these deeper carious lesions, and perhaps would require some layering at the deepest extent of the cavity to reduce the effects of polymerization shrinkage and transform the cavity into the classic saucer geometry you advocated for a decade or so ago. I love the 8833 diamond and the emphesis on creating compression joints. I will, however, say that I find the use of the 6862 diamond to be puzzeling. Now that we have diamonds that cut Calla Lilly geometries, like the 8833, why use a flame diamond? Its shape is diameterically opposed to the shape you're trying to cut. I'd suggest a 392 "interproximal" diamond; the one made by Komet is too short and has a flat end, but I've used ones made by Diatech (part of the Coltene brand) and they make them longer and with a beveled (chamfered) end. They are much thinner than 6862 and have the right shape. Most often, they can do the job of that long FSD3 needle, which seems to be your new "workhorse" diamond, and it cuts more efficiently. I also don't agree with selective caries retention, especially these days, when access to bioceramic materials and our understanding of what makes vital pulp therapy work or fails has increased so much, but that's a topic for a different day. And alas, while the information in the video is solid as always, the microscope closeups are really hard to watch and are off-center as everybody commented.
