It is important to only use the term dysplastic nevus when the lesion shows the combination of both architectural disorder and cytological atypia. Many other nevus variants can show cytological atypia including neonatal nevi, nevi of special site, scalp nevi, nevi of the ear, recurrent nevi etc.

Primary cutaneous secretory carcinoma is exceedingly rare. Before making the diagnosis it is important to make sure that it is not arising in a salivary gland or breast. Although considered to be a low grade tumor, a recent publication where a patient developed nodal and lung metastases challenges this supposition. Most tumors are associated with the ETV6-NTRK3 fusion gene.

Porocarcinoma is the commonest adnexal carcinoma. The important histological features include by definition, origin from the epidermis and the presence of intracytoplasmic lumina or ductal differentiation. Squamous carcinoma with vacuolation is the commonest differential diagnosis and this can be excluded with EMA and CEA immunohistochemistry.

Clear cell hidradenocarcinoma is an exceedingly are generally high-grade tumor. It may arise in a pre-existent hidradenoma or develop de novo. The latter can be difficult to distinguish from clear cell SCC, trichilemmal Ca, clear cell melanoma and clear cell metastases.

For the Residents and Fellows. Four cases of carcinoma arising in a background of spiradenoma are presented. The concept of low and high grade variants is discussed. I am not entirely comfortable with grading these tumors, perhaps a reflection of personal experience. I will be very interested to know how you all think. I may have to mend my ways!!!
