Maybe you had a claymore (or other mine) that downed an enemy while you were shooting a different enemy. You see the "downed" hitmarker and think the guy you are shooting was downed, but he's actually not downed yet. Then when you finish the fight you see the enemy has a different name because it's a different person. Could also happen if you used an airstrike to down 1 enemy while you fight a 2nd enemy - if you down enemy 1 while fighting enemy 2 you might get confused.
