This can be done in Chapter 19, right after run up the stairs and shoot the guy with the T-Bolt Sniper. I rotated between Save Games 1, 4, and 5. Save Games 2 and 3 were taken from me playing on Hard difficulty. Anyways, the video pretty much explains what you should do when going for the Survivor trophy.
