Wow, After go’ogling Simmy Krotiel’s latest I noticed it had undergone revisions to add 4 ins. and even more using accelerated growth techniques, after 3 ins. more I feel so huge now, thanks
After REZUM, what happens to previously installed Urolifts from 11 years before? There's no median lobe. Also, how well this work for a 62-year-old patient with a mildly trabeculated bladder for & slightly displaced or elevated or whatever it's called bladder neck?
VA endocrinologists should be embarrassed for themselves. I had a very post menopausal endo attending try to tell me what I SHOULD feel like as an early middle age male, and then went on to blow smoke up my ass telling me I should feel perfectly fine at a T level of ~200 (and while significantly symptomatic). TRT is LIFE CHANGING therapy, and I probably would have suck started a shotgun after getting out if it weren't for the TRT I paid out of pocket for. VA endo and endo in general = clownshoes.
So glad I found this channel. As an MS1 interested in urology I hope to find a supportive mentor in the field

Love the discussion! Two great respected minds excellent questions! I am curious to find out as your discussions around collecting data from focal procedures, why don’t the speciality clinics like Sperling Medical whom have performed thousands of focal ablations, are they not required to provide follow up data to the academic institutions so that the knowledge base for cure or recurrence can be updated and allow for more widespread acceptance of focal offerings. I am in the process of deciding on a focal laser or cryo procedure. I just turned 65 so I learned that I would be covered under Medicare, vs going to Florida for an out of pocket expense.
How can you know if there is hypermobility before the surgery? Or will you find only after?

From this patients perspective, Nomograms are old and tired. It’s an outdated approach by definition as it doesn’t use all the latest information or data at hand.

A 1 centimeter margin? SO if there is a 2cm tumor, that means a 4cm total area. About the size of a Golf ball. That seems like there isn't much Gland left after that.
Hey docs! Thanks for the great info. I’ve had IC for 14 years, and recently had to have three embedded calcium oxalate bladder stones removed. Have you found that bladder stones and IC are connected? Perhaps oxalate irritation of the bladder wall?
PSMA has limited utility in garden variety recurrence (wherein salvage should begin prior to .2).
HiFu is covered by Medicare since Jan 2023. Many private insurance now cover it.

can a person take DHEA inserts while on Estrogen and progesterone?
This is good. Prostatectomy 8.5 yrs ago 4+3, post op svi..about 30 psatest ranging between .02 and .04...now...suddenly .08. Psma scheduled...no mri...radiation here I come? Recovered great from surgery...consultation? that was my urologist weak point seemed tight liped. Shoulder shrugging and a lot of I don't know to my questions...now have a new urologist and first visit I get the .08.
Just had my first post op psa almost 3 months after operation my stats are same as yours but psa .01. Curious what they told you would trigger radiation? Was it change or rising above an absolute threshold?
@jazandriz psma was negative so continuing psa every three months . If i have anymore rise then salvage radiation in region....or another psma....? Not sure kinda in lala land. I've really restricted my diet to see if it stabilizes or drops any. If i get another rise then something in the move. Just random salvage may not help but maybe my only option unless it gets pinpointed with psma.

I have to be honest, after watching this, if Dr. Pearlman ever relocated to my city or if I ever relocated to hers, I'd want her as my urologist. She sounds really open-minded.
