Top 10 Tips for Time in Range: AID Systems for T1D
1. Food and insulin: 3 balanced meals with 10 minutes activity after = (minutes 0-4)
2. High fat and protein meals: “Find out before you fiddle” then 25% extra insulin increments = (minutes 4-10)
3. After meal spikes: Insulin 15 minutes before in the abdomen or arm = (minutes 10-14)
4. 3 every 30: 3 minutes of movement every 30 minutes sitting = (minutes 14-16)
5. 10 by 2: 10 minutes of activity drops the glucose by 2 mmol/L between meals = (minutes 16-22)
6. 50/50/20: Insulin reductions for pump therapy = (minutes 22-30)

    Hi John, I wanted to say a huge thank you for sharing this resource with the rest of us. GNL has been the key in keeping my 4 year old newly diagnosed T1 daughter in range for the first day since her diagnosis, two weeks ago. It's a total game changer. I understand if you're not answering questions given the nature of your blog, but I wanted to ask what the "half plate of veg/quarter carbs/quarter protein" looks like at breakfast. I assume we're in a fruit and yoghurt space with some porridge/granola and maybe an egg? No worries if you don't do questions; the blog and videos have changed our lives in the biggest way since the diagnosis.

    Would it have been an option to reduce your basal prior to starting in addition to the bolus reduction?

      it is certainly an option but for me remembering 90 mins before is not realistic also when in 90 min of bolus insulin, a basal reduction does not work effectively if i was not eating within 90 min a basal reduction would have been a very good idea.

    One of the best explanations of how fat reduces insulin resistance. Thanks. If a person eats carby foods earlier and fattier foods later, will that help reduce overall insulin requirements?

      Hi James, thanks for the review! In response to your question. The main reason people are more insulin resistant in the morning is due to two things. 1. Higher levels of cortisol and adrenaline causing 2. The consequent higher levels of free fatty acids in the blood that translate into more DAG’s in the cells and therefore insulin resistance after the overnight fast. So, if you consume a high carb lower fat meal first thing, this will lower the free fatty acids and DAG’s for later in the day - but it will be hard to keep post breakfast glucose levels under control - the need for activity after eating or using GAME from dynamic glucose management is essential. After getting over this it should be easier to manage lunch and evening meal with carbs with less insulin. If you start the day with a high fat meal it will make the DAG effect more pronounced. This is ok if you are following a low carb approach as you will not have a glucose load to manage later in the day - control will be easy. However, if you throw in a higher carb meal at lunch time you will have a hell of time overcoming the DAG effect. I cannot answer directly will it lose insulin needs as it depends what the rest of the days meals look like. The above might help you think through it. My best advice is to be as consistent as possible with mean types - if going low carb, stay low carb for the day and don’t switch to high carb. Vice versa

    Here's another tip: take the peel of the sensor sticker off before inserting the sensor, when you pur the sensor in the applicator take the peel off the sticker, when you insert the sensor it will be already sticked to your skin.

    My sensor fails within a few days. I can trick my dexcom and the numbers are always right on with the dexcom. The accuracy isn't good with the guardian sensor.

    Hey John, I have been struggling with where to put my CGM because I'm very lean. I'm thinking this location might work for me, but how to you avoid having your waist of your pants pulling on it there? Any suggestions?

    Well I'm not big on Medtronic CGM, but I figured I would give you and you viewers one more tip. Buy the Skin Tac in the 4 oz bottle. I bought mine March of 2014 and still have 75% of the bottle left. I also use it every three days with my infusions. So it lasts a long time and is much cheaper in the long run. www.amazon.com/Skin-Tac-Liquid-Adhesive-Barrier-SKIN/dp/B008IUA6DU/ref=sr_1_5_a_it?ie=UTF8&qid=1482373936&sr=8-5&keywords=skin+tac I will warn against the skin tac remover if you have sensitive skin, it doesn't like my skin and leaves a rash. I will use isopropyl alcohol. Anything over 91% works good. Just use a brush and wipe it on, let it dissolve into the patch while you prep your new site. I will brush it on twice over 1 minute and the old infusion practically falls off, and it's easier to clean up also. Good luck, I hope the skin tac works for you. It works great for me, it the pads are expensive compared to the bottle that seems to last years.

    Interestinly, my endocrinologist and CDE told me to have the sensor sideways (landscape), but what you said makes sense. BTW, love your bum : ) I just migrated from my old pump to the new 630 today. Not sure what kind you have, but nice video.

    i started getting an allergic reaction to the tape under the transmitter. Do you have any advice on how to prevent this.

      Alison Murray you can use tegaderm and cut a hole where the sensor goes through. Skin tac provides a barrier as does a product caller cavilon or skin soft. Hope that helps

    I don't know that I'd want mine on my butt...they say that laying on the sensor can give inaccurate readings; what about sitting on it? And isn't it awkward to put it there where you can't see as easily?

    Thank you for this video. My daughter has just started trying with the enlite cgm and our first sensor was giving errors and was taken out of her belly within a day. I wasn't sure where else it could be positioned, but seeing this has given me confidence to try alternate sites.

    Will the weights affect BG differently in every person?

      Hey, generally weights alone will cause BG to rise if done heavily, due to adrenaline causing the liver to release extra glucose. Depends if you have a lot of active insulin from a previous bolus in the last 90 mins, as that could mean you go low. I tend to make sure I have bloused at least 3hours prior to the session so then can see what is needed. Measuring your BG will let you know what you need. Before, during and after.

    Hi, I got my 640G 2 weeks ago and the guardian 2 link and sensors 1 week ago. I had so much trouble with, big gap between metter and sensor and it dies after 3 days. I put a new one and the gap still there. I will try your tips for the next one and I hop it will works better....

    Just curious how you get your sets to stick to you when working out so often. When I work out my sets seem to lose their "stickiness" and start peeling off. Just curious. How many times do you eat a day when trying to lose weight?

    Hey, I do 8 to 9 as our driving regs say must test BG before each journey so as drive a lot for job then have to do lots extra BG as they would not accept SG. Also as I do lots of exercise sometimes the drops are rapid so although the SG is a good guide I need also the BG to prevent hypos. Hope this helps

    Just curious. The Veo for me is always 'weak signal' or 'lost sensor' if the pump is on the other side of the body whilst sleeping. Is the 'new' sender having a better connection as it is a new generation sender? Also the battery life is always suggested 30 days on the Veo but I usually had 4 - 10 days while using the sensor though Medtronic claims no one else has this problem. In fact even without the Sensor I have never had a 30 day battery life. Speaking to others I have found it more common. I know the new 640 is 2 AA instead of 1 AAA Curious if you notice any difference in signal connection/loss?

      A Pumper Hi A Pumper, Yeah I used to get quite a lot of lost sensors on the VEO too. The new alarm ion the 640 is "cannot find sensor signal" and since I have had it I have only had one of these alarms which required me to go to "re-connect sensor". Therefore so far there seems to be a much better connection from my first 64 day experience. The battery life seems to be about 14 days with the sensor running full time. As you can imagine with colour screen, many more functions and I have it on vibrate that I am draining it heavily. One thing to note is to ensure the "Backlight! is only set to 15 seconds and no longer otherwise the battery runs out quicker. Thanks for watching, I hope you found it useful.

      Very useful and thanks. I am up to get my new meter in the next few weeks. I must also ask, as I have been used to using about 8 -10 finger strips per day. However the Dr here is trying to get me to use MUCH less even though my insurance pays for it. Have you always used 8 or 9 per day and does NHS pay for those? Or do you go private insurance? I was in the UK for some time and now bing in another country it is done differently. I was really curious if your Dr and team find the 8 -9 strips per day 'normal' for someone on a pump and sensor. They are claiming since I have a sensor that I need less than half of this, but brittle and trying to keep my numbers down it is hard to manage with less. They have never been so strict and I have had 850 strips per quarter for years. I am trying to prove that I am not the only one with a sensor needing more than the standard patient, especially being brittle. Again many thanks
