You know just because this wonderful Dr can can do a procedure that is against what your moral stick up your ass is wrong or is doing this for anyone that doesn't want it. In fact I am thankful for the technology and the brilliant Dr's that is capable of doing this . They are truly life saver to a lot if not all Transgender people who are human beings also who wants nothing more than to live a happy life just like everyone. I mean if this is such a rule breaking thing against your beliefs why even bother to watch it ? Really. Correct Dr Mahtre has done some targeted studies: 36 patients were followed for 9 months to max 7 years (only 3 patients) for procedures performed on patients with congenital blind vaginal pouch and 46 XX pelvic anatomy that did not require the more extensive dissection of SRS. Long term strictures were not specifically followed, long term dilation not required since the peritoneal lining met with congenital vaginal lining, and max depth obtained was 7-8 cm. This is a promising study, but more studies with higher Ns, longer follow up to provide information on the outcomes for transgender, intersex, gender nonconforming patients that are interested in peritoneal pull through option for vaginoplasty.

When Dr. Badran begins on the external genitals, it is clear the patient lacks even the vaginal introitus. Dr. Badran begins with an incision to open an introitus. He then opens the space for the vaginal canal until the dialator being used can be seen on the laparoscope pressing against the peritoneal pouch. This provides a visual location to open the peritoneal tissue inside. The peritoneal tissue around the incision point is mobilized. This does not appear to require a large area. After mobilization and opening to the new vaginal space, the peritoneum is grabbed and pulled down to the neo-vaginal introitus. The peritoneal tissues are sutured to the introitus. Then inside the laparoscopic surgeon uses a stitch to gather and pull the anterior of the neo-vaginal canal closed. This closes the peritoneal cavity as well. This case may be of particular interest as the patient has no existing vaginal introitus and therefore no existing depth to work from.
