Probably I missed your disclaimer that all glaucoma is not equal. There are diverse types of glaucoma based on cause, mechanism, and complications. The most prevalent one could be pseudo-exfoliative glaucoma which has little to do with intraocular-corporeal fluid dynamics. Though the video overall touches on a very delicate issue for scientists to think of.

If the crux of this argument is lowering the IOP (relative to the ICP) in Normal Pressure Glaucoma, why not use an eyedrop at bedtime to keep the IOP 'suboptimal' in this subset of patients, say around 10-12 mm.Hg?

Some studies claim that if FLACS is used for the surgery of posterior polar cataract, it creates a cleavage plane between posterior capsule and epinucleus and hence facilitate the removal of epinucleus without rupturing the posterior capsule . Is it true ?

No mention of the Light Adjustable Lenses?!… I just had them put in a week ago. I can’t believe the clarity of night driving. In a couple weeks, I will go in for my first adjustment. Right now I’ve got perfect distance and mid. I’d take these lenses before anything mentioned in the video.

Toric&monofocal + glasses seem to be the best solution. trifocal, edof, etc. will not get you a perfect vision, even worse you cant improve with glasses once you got these lenses implanted. Maybe accommodating lenses will work future will show.

Many thanks for a great presentation. Not clear though on the situation with accommodating lenses. I understand there is a lot of potential and they haven’t been approved yet (or have they by now?). But can we get some more insight into how they perform and whether it would (or not) be a good choice?

Its interesting that the word "Tecnis" and the sub heading are legible on the slide but the content needs binoculars to read. Whats really interesting is that these eye doctors dont seem to have considered the quality and delivery of their presentation based on the fact that its our ability to see that they rattle on about, as the old saying goes "cant see the wood for the trees" "Let me just compare" That made me laugh outloud!

Im an electrical engineer with glaucoma. The focus on IOP has always bother me. The fact that it needs to be compared to something, I was thinking arterial blood pressure at the entrance to the eye, is in the damn obvious class. The association of glaucoma and IOP goes back an easy 100 years and only now somebody thought about this is very disappointing, Congratulation on being the one. Let me add that the arterial pressure has to be greater than the ICP if blood is to get to the brain and above both ICP and IOP to get into the eye. Low ICP permits lower arterial pressure while supplying the brain with blood hiding low arterial pressure against IOP that otherwise would have caused fainting. I am also concerned that lowering the IOP against the ICP would cause the eye to dis-inflate, causing it to change shape and screw up vision. I have tried and rejected some eye drops because I lost visual acuity. I'm wondering if it is because they worked too well and my eye changed shape because if dis-inflated. Congratulation!! and good luck.

I use such lens. I was very dissatisfied with the quality of the lens. I am 40 years I have no complaints against the doctors. For 65+, a good option is possible. But for those who are younger, it is better to install at least Symfony and Synergy. With Eyhance you will suffer greatly with far vision or wear glasses. It's a pity that the lenses are put in actually only once in a lifetime (only in super emergency cases you can change them).

In 2022 Isn't there high lens replacement with panoptix? Some drs. I think don't use synergy much anymore? With eyehance what percentage is getting intermediate at 24 inche vs farther away.. What size font are people on average able to read (though everyone different) with eyehance. . I heard some doctors do mini mono with eyehance?? Does that mean they start one eye with .50?? Can you get roughly the same results with mini mono intermediate/distance with monofocal lens. What is average that people who just do plan distance get lucky and see computer clear at 24 inches. Is that very rare. Thanks

Great hangout! Thanks to all 3 docs for the informative and enlightening discussion. I have 2 questions to ask: 1. Can the halo/glow induced by the trifocals/multifocals be reduced with polarized glasses. 2. I heard one doc say that with the Panoptix lens, only 92% of the light enters the eye. Is this true and if yes, will brighter lighting compensate the 8% loss?

    • @--2455

What is true is that Panoptix shows difficulties in dim conditions. As reading menu in restaurants. Consider that some retina issues reduce the light reception: you wanna avoid Panoptix if you have or potentially have in the future (ex relatively young patient with high myopia >10 diopters) a retinopathy
