MyRenalCare is web based Electronic Patient Record (EPR) software for patients with chronic kidney disease and providers who deliver their care. Developed in collaboration with the Wessex Kidney Centre at Queen Alexandra Hospital in Portsmouth, it is an effective remote monitoring platform enabling supportive self care. "MyRenalCare allows me to monitor me, ands my team tom monitor me, remotely" thereby transforming the way individuals with CKD manage their disease.
