The Utah Children’s Care Coordination Network (UCCCN) supports and promotes care coordination for CYSHCN in primary care practices, public health, and other organizations throughout Utah. UCCCN offers information, resources, opportunities to improve skills and learn about tools, and support in managing difficult cases and connects the coordinators through a state-wide network of peers and potential mentors. UCCCN supports its members in helping families make informed decisions, access valuable health and social services and resources, and improve child and family outcomes. Meetings are held monthly; recordings are available on the UCCCN RUclips channel.
