Sir, as pt is under GA or spinal, do we still need to infiltrate LA subcutaneously for block to be effective Or can directly proceed to 2-4-8-10 injections? Also, can we reduce the volume with higher concentration of LA or is it a volume based block?

Absolutely right, sir. For Bilateral TKR , same incident happened yesterday. On left side perfect pattelar snap & other side I had to struggle to get desired response rather sartorius contractions was repeating.
