Way back in SS arc during Zaraki's flashbacks. He named his vice captain Yachiru based from the person he admired the most which is Unohana Yachiru. The title Kenpachi is obtained by killing the current holder of that title.Thats the tradition of squad 11 thats why when Zaraki killed the previous captain of squad 11 he got the title Kenpachi and became the captain even though he never went to the academy. Thats why even though there were a lot of Kenpachis that already went by, Unohana is still technically the official holder of Kenpachi even though she let go of the title and the squad 11(which youll now why next ep) cause no one was able to kill her so she couldnt officialy pass down the title. It may seem dumb to a lot of people that someone has to die but they ALWAYS forget that characters in soul society value their morals and pride so much that they are willing to do anything to keep it intact.
