    Firstly, some big blind spots among these experts. Where are the Teeth, the Mucosa, and the Thyroid segmented? Why Teeth? They are there and future dental work is likely. And they are surrounded by Mucosa except when buried in Mandible or Maxilla. Why Mucosa? It's the major OAR in the H&N! There is no anatomy called "oral cavity". The oral cavity is a potential cavity lined with mucosa and does not include the muscles of the tongue (if you want to contour a Tongue, then contour a Tongue!). In addition, if you abhor 'creative contouring' like I do, the sublingual glands and pharyngeal constrictors are so close to Mucosa that their radiation avoidance is realised in successful Mucosa avoidance. Why Thyroid? We cause hypothyroidism commonly enough. Secondly, my appreciation of the problem with organ contouring is that there is no advice on how to do this for the "non-experts". Much of the contouring shows that some people just can't be using their eyes, and/or don't vary their windowing to actually see what can be seen. But there are also important questions which the experts have not agreed on yet like: a. when you contour a Parotid, are the vessels in or out? b. when you contour the Larynx, what should be included? Epiglottis, LaryngealCartilage, Subglottis, Arytenoids, Hypopharynx. Thirdly, I would love to have the resources of Groningen, but I don't. If the aim of the game is to influence the variability in OAR definition in the general population, then perhaps there should be demonstration of how to perceive the organ boundaries accurately, as well as kept to the lowest common denominator - the non-contrast CT. I would prefer to see the discussion look like Pixel Wars, 'fighting' over whether a pixel is included or excluded and why, and then reaching some better rules for contouring for the non-expert RO. When attending previous contouring sessions at ASTRO etc., it has been very clear that the "experts" weren't actually doing the contouring. Once again, we don't all have residents to do our work (and I wonder how many of the residents who do this contouring work, actually end up outside as non-experts??). Fourthly, until 'manual' contouring is based on pixel values (e.g., Parotid CT#960-1040) how is the variability ever going to be reduced?

      Thank you for your interest and input on this case and our C3RO process. You have raised a number of additional points of controversy re H&N OARs. Of note, we only included a few OARs in this exercise due to time limitations, and agree that in general additional OARs should be contoured for routine practice (i.e. oral cavity).
