I got a feeling Hanks and the writers got some inspiration for these intros from Rod Stirling's intros on the Twilight Zone especially the repetition of the title of the series at the end.
The art department of star trek tng/ds9 and voyager and enterprise (Denise & Mike Okuda, Doug Drexler et all..) played this Tom Hanks mini series often in the office while working on star trek in the 90s.
Episode 49 intro: “Food was important to the men and women at the Kennedy and Johnson Space Centers as they worked tirelessly to support the various projects of the Space program. In this episode we’ll cover the restaurants, the grease trucks, homemade meals, and On-site cafeteria food that would energize the workers who would help the astronauts get from the Earth to the Moon.”
I love that relief sculpture of Apollo so much. I can’t find the artist or any other instances of it being displayed apart from this series.. it’s perfection though. Oh yeah, and Hanks is the devil @ric david
I've got the DVD set of this series, watched it numerous times as i love all this space stuff, and yet - not until I watched this video did I realise Tom Hanks finished all his introductions with "From the Earth to the Moon". The artwork is of the god Apollo on his chariot. I can't think of ever seeing this particular artwork anywhere other than in relation to the Apollo space program, although I could be wrong there so don't quote me on that. Your best bet to find it or something similar might be to search Google for "Apollo on his chariot", and then click images. Hope you find something to your liking.

Found it: the artist is Michelle Millay, she does a lot of work for tv and film, did a lot of set stuff for Pirates of the Caribbean. Death to Hanks.

MASJF if I recall, Hanks did not do an intro for episode 12 (because he co-starred in that episode, the finale.)
Blythe Danner narrated the Intro for Episode 12.
Commander Jim Lovell was not available to do the introduction. He was at the opening of his son's restaurant in Lake Forrest, Illinois.
