Here is what I did different from my first playthrough to achieve this ending: - I did not do a full playthrough, I just loaded up my recent save before playing Mary's tape. - I frequently viewed Angela's knife. - I viewed Mary's picture during her monologue in the long hallway before reaching the rusty stairs. - I did not fire a single bullet at an enemy.
