Actually, a four hour erection (or longer) can cause a condition called priapism. It's a medical emergency and can result in damage to the corpus cavernosa (the two blood engorged areas on either sides of the shaft). Depending on severity they can use drugs to suppress the erection (Not sure the details here). In some cases it becomes extreme enough that clots can form in the corpus cavernosa: This requires at the very least drugs to dissolve the clot and possibly a shunt to drain the blood (that's right, imagine a needle having to be inserted into your dick). In extreme cases necrosis can occur which would require amputation. Originally they said something to the effect of: In the event of a four hour erection seek medical attention immediately; later they changed it to "to avoid permanent injuries, seek immediate medical attention for an erection lasting more than four hours". If they said "Seek immediate medical attention for an erection lasting more than four hours as severe injuries could occur requiring the insertion of a shunt, and even gangrene which would require the amputation of the penis", pretty much nobody would buy it. People would probably be saying things like "Holy shit!" and "Fuck that! I'd rather be limp". At least I can pee with it!
