This was actually the first time Wanda realized what she had become. She was reminded of it when Amerika showed her her kids in the other Wanda's house. She viewed Monica as an intruder even though Monica helped Wanda give birth. When Wanda saw her kids reaction to her in that other Wanda's home, she knew that she had become the monster and the intruder. That's why she reacted the way she had.
