    What happens when you have progressive saddle numbness but are deemed not to have cauda equina syndrome, no metabolic syndrome, no injury, no verifiable disease by blood test or spinal tap, and only abnormal thing on 8 different MRIs is a very slight disc bulge at L5-S1?

      how are you now ? I am having the same issues ? saddle paresthesia and does not look like cauda equina syndrome and mri confirm disc bulge at l5-S1 on right nerve root ?

    Interesting I have never seen this technique before, usually they tell you to put a pillow between your legs, which I've never understood how that aligns your spine.

    Love this guy so far, but I don't understand the conclusion of his thought experiment here - that a new etiology discovery will not suggest any changes in treatment. (Although I do understand the lessened use of an etiology for complaints we already have successful treatments for.)

    The mechanical level, outside of a clearly identified trauma, is almost always downstream in the causal chain. The first domino is usually metabolic syndrome; either early or advanced. Grain(= sugar)based diets rot more than your just the teeth. That is what's driving $50B+ annual health care costs in Ontario alone.

      I'm not Dr. Hall but I will respond. There's nothing complicated here. It's very simple. The example in the video is when a patient answers they can't work b/c their legs give out (leg pain). Well that doesn't match up with the answer the patient gave to Q.1 which was that their back pain is worse than their leg pain. If there back is worse than leg pain then the answer to Q.5 should be they can't work b/c of their back pain.

    Thank you this was very helpful, just yesterday I saw a patietn with 3/53 hx of progressive lower back pain, and I was trying to make my history as thorough as possible, I picked up some helpful hints from this video. I did ask about red flags for back pain. Anyways it turns out my patient had compression fractures in L1 L2. Very helpful video!

    I didn't quite understand why my GP didn't send me for an MRI 'right away'. After reading Dr. Hall's book, I understand her sequence - she basically followed what Dr. Hall said - and it makes perfect sense to me now. First, a few months of physiotherapy and exercise was prescribed but also 'the usual' SLR, reflex, and EMG/NCV checks. When all of those tests indicated issues - (no left ankle jerk with a strong right response), a very awkward toe-first gait, a 'very loud' EMG, inability to do even one left calf raise or hip abduction while normal on the right side, limited SLR, and failure to respond to conservative measures, then she sent me for an MRI. At that point, I think it was a case where a surgery was almost a certainty and I am pretty sure I met Dr. Hall's criteria. The surgery reduced the leg pain and almost eliminated it but not so much the back pain - exactly as I expected. The back pain is subsiding slowly with exercise and PT.
