Who is angry in the October 2024 AKT feedback? What we need to know!
This is a must for anyone sitting the AKT in 2025. With the AKT feedback report for October 2024. There are three key areas that I think need to revising:
1) Prescribing - Being able to revise adjustments for the elderly and also side effects. This could be a 'quick quiz' on any debrief
2) NEUROLOGY - BIG hint. This has been fed back on the last FOUR feedback reports and so should be the top of revision. Ensure that you are confident in all the basics for diagnosis.
3) New asthma guidelines. Asthma guidelines crop up time and time again. As the guidelines have been changed then this is PRIME for a question.

After every RCGP AKT exam there is a feedback report. These are really useful as they give a good idea of what MAY occur on future exams. The overall advice though on the exam is really useful to go through in any case. If I have a trainee going through their AKT, I will usually give some time to going through this.     I will admit I was skeptical when I first saw this. I didn't think It would help with my clinic times and I thought it would probably slow me down. But I am a convert, it makes life so much easier my consultation times have sped-up, The more I have used it the easier it has got, I am probably not using it to its full capacity yet but I am working towards it. I am leaving this review using my Lexacom® Echo

    Really helpful. I have been looking at this for my PCN as well and trying to work out where it would be most of use. One thing that seems frustrating is that you can't apply rules that have been agreed at practice-level to a whole practice inbox rather than just your own results. I think that would be a gamechanger...

      Thanks for the comment. This is something that I have played with. I think you can apply the rules to the whole inbox though. Once you are logged in, instead of clicking on 'your list' (far left) bring up the practice list (middle) and then apply rules. I am still early days and very much experimenting, so happy to investigate! I am in a similar position to you that I want to do a Federation rule set is the ultimate aim and will share as I go. I am hoping to do a video to document the cost saving (which I think can't be underestimated!) plus the reduction in decision fatigue. Anyway, please keep me updated with how you get on.

    Thank you for this overview. What happens if you have set up the rules and later your local lab changes their testing method and the normal ranges slightly change?

      Superb question - in short this is one of the risks. I think though there would be a good mitigation for this by either tightening the rules (I.e. what you feel is normal), having other safety checks and so limit the rules to only really safe areas OR (think gold standard) is to try and get update from labs. Hope that helps. You are not the first to ask and I feel a follow up video brewing!

      At present the only ones that I know are the Ardens ones that I have done a short video introducing it! ruclips.net/video/LXUxR56uLGQ/видео.html From discussion on other social media platforms, it looks like larger networks (PCN, ICB level) are creating local ones that can then liaise with local labs to ensure that nothing is missed. I have a role with our local federation where I am leading up on this to see what we can do! I hope that helps.

    So true Dave. as a young GP I looked at a practice, and discovered 6 months ago a GP had been 'kicked out' - the job was to replace him. I tracked that GP down and interviewed him! Boy oh boy, I'm glad I did. I went into an interview and spotted all the things he talked about: I'd have hated it there. I would also add: try and become a locum there. How they treat you as a locum will tell you about the 'culture' at the practice. Try and talk to the practice manager alone, and ask a few good questions - they are often very open. I raised come concerns about one practice I was being interviewed at. If a Partner position: ask for the accounts! I took them to a private accountant who knew GP accounts for his assessment [cost £200]. well worth the effort - how they spend their money will tell you their priorities, what every they might tell you. Look for a 'good fit'. One practice was subsidizing the NHS to the tune of £10 per partner - they were older, adn the money was less important to them than enjoying their job. With a young family and large mortgage I disagreed! Finally, if you don't success keep applying. BUT after a 'sorry we don't want you', try and talk to partner as to 'why. You will learn much from this, and decide what you need to change, or it was 'them not you!'

    can you import a whole questionnaire a patient has completed into a word letter?

      Depends on what type of questionnaire, if it is the last consultation then you can create a letter that pulls the last consultation across. If it is a specific one, then if it is linked to a read code then you should be able to pull the last values across What type of questionnaire was it in particular?

    I logged on a couple of days ago and lexacom threw up this ECHO registration page. I had no idea what it was. Quickly registered with my nhs account and got cracking with work. I have NEVER been so excited to get back to work so I can investigate this further! I'm having dreams of dictating patient notes all day. My fingers rejoice at the thought!

      Very similar to a lot of other social media sites. A central feed that is a mixture of adverts and posts from other medical professinals. There is a clear 'work' element to this. You can then customise what you see based off your clinical background. There are separate tabs then to access the other functions like the surveys. I hope that gives an outline!

    Is there a way to run this when filing the pathology when the bloods are autofilled

      Great question. As long as the protocol is set to ‘run when patients notes opened’ then when you are filling it will work. I am conscious that it may slow people down when filling so worth discussing at your practice. The other option is that would you could use the searches to run a catch up when you have time. So we have had a huge catch up with our pharmacy team adding the codes, but despite this we have now 10 patients come through. I hope that helps

    Thank you for this video, a very clear explanation. Can I ask whether a test with lower sensitivity would also correlate with a lower PPV, or a higher specificity test would correlate with a higher NPV. I have tried to calculate the PPV for the COVID-19 tests in this video and come up with 92%, but with a NPV of 99%. I am feeling very confused as to why, and not sure whether I have calculated wrong?

      Oooh, good question. The two are linked, but not necessarily in that way. You are looking at two slightly different aspects. Essentially 'how much do I trust a positive test' PPV vs 'how good a test is at picking up a disease'.     Hi Dave, I didn't know you could do that. I shall have to see it will work with my community dermatology workflow in Darlington. Just one question, can you compress the image with this workflow? I need to be retrieving lots of images when I review dermatology patients but if the file sizes are more than I think 1.5mb it takes time so I am always resizing the images in windows photo app, renaming then uploading into record attachments. It slow and clunky but it gets the file size down to around 250-500 kb which then download quickly when I need to review them. Cheers. Martin

      I am very glad that you found it useful. I just checked on the example. When I sent it as a 'link to outlook' it comes out as a 739kb file (.TIF), which maybe suitable for your needs.     Aside from potential "bad news" results, what other rationales/justifications in your experience would there be to hide these entries? And can these things be later un-hidden on the record? Thank you for your time.

      The big one for me would be safeguarding, apart from that, I try and minimise 'hiding' things from patients as it just creates a lot of issues. You can use the same method to 'unhide' the consultation and result by the 'right click' over the consult and then selecting the appropriate response. I hope that helps!

      @@anjupatel2835 yes I would! Save the work for everyone. I am trying to think about what would useful. Do you think a simple ‘this is CKD x and not coded….’ Would suffice or whether it should go further and go into reviewing medication (ACE/ ARB and statin)?
